SCOPE 1.1 The Quality Assurance Program descriaed herein applies to the use, maintenance, anc repair of shipping packages for special form radioactive material.

The program supplements the ITT Grinnell Radiation Safety Manual submitted in

'accordance with 10CFR30 and on file with the Division of Materials Licensing.

It describes the procedures, safety precautions and radiation surveys taken during handling, storage, and shipping operations for shipping packages for special form radioactive material.

The program identifies the Quality Assurance Organization, methods and techniques for the ef f ective control and assurance of radiological safety and' package performance, and to ensure that all necessary radiation surveys, quarterly maintenance, leak tests and equipment inspections are performed.

This program is in accordance with the require-ments of Appendix E 10CFR Part 71.

1.2 ITT Grinnell Corporation is a license-user and does not perform any of the following functions that apply to shipping packages:

Design, Fab-rication, Assembly and Testing.

Therefore, these items are not addressed.

To assure that all radioactive material shipping packages are designed, manufactured, assembled and tested in accordance with a Quality Assurance Program in accordance with 10CFR Part 71 Appendix E and approved by the NRC, ITT Grinnell Corporation shall obtain a certification from the supplier to this ef fect.

ORGANIZATION 2.1 Chart 1 identifies the reporting functions of the ITT Grinnell Corporation RD&E Division Organization with respect to the Nondestruct-ive Testing 3ection.

2.2 Vice President:

The Vice President, Research, Development and Engineering Division is respon-sible for the over-all administration of the Research, Development and Engineering Division and responsible for compliance to contractual and functional requirements and Nuclear Regula-tory Commission and State Radiation Rules and Regulations.

2.3 Nondestructive Testing Manager:

The 3 1estructive Testing Manager is administra tively re nsible for the entire Nondestructive Examinat

_ Program.

He maintains constant surveillance of tLe Radiation Safety Program.

All purchase orders and revisions thereto are approved by him to assure all radiation related equipment meets the requirements of the specific regulatory code.

He is responsible for scheduling all internal program audits to insure program compliance.

The Manager of Nondestructive Testing together with the Radiation Protection Officer provides protection against the hazards of radiation and approves all measures used to safeguard personnel and equipment.

2.4 Corcorate Quality Assurance Manager:

The Quality Assurance Manager is responsible for conducting internal corporate audits of the Radiation Safety Pro gram.

Non-compliance shall be reported by him to t he Vice President, Research, Development and Engineering Division.

_ Radiation Protection Of ficer:

The Radiation Protection Of ficer assists the Manager of Nondestructive Testing in the operation of the Nondestructive Testing Section.

He is directly responsible for all matters relating to the Radiation Safety Program.

He maintains all records applicable to the Radiation Safety Program, prepares Radiation Safety procedures for approval by the Manager of Nondestructive Testing.

He is responsible for per-sonnel training and auditing radiographers and radiographer assistants for program compliance.

Periodic inspection and maintenance of all radiation equipment, care of radioactive materials, audits, surveys, and complete compliance with safety requirements are within his j urisdiction.

2.6 Field Site NDE Supervision (' Radiographer):

The Field Site NDE Supervisor acts for the Nondestructive Testing Manager at the Field Site.

He supervises all activities of Field Site Nondestructive Examination.

Maintains control of all radiation safety records and is responsible for distribution of all records to the Radiation Safety Of ficer on a weekly basis.

He is responsible for the Radiation Safety Program at the Field Site.

2.7 Senior Test Technician (Radiocrapher):

The Senior Test Technician perIorms Nondestructive Examinations and re-ports to the Manager of Nondestructive Testing on the progress of trainees whom he observes and instructs.

He also assists the Radiation Protection Officer in the care of radioactive materials and assuring compliance with safety requirements.

2.8 Radiographer

The Radiographer performs Nonestructive Examinations and reports to the Senior Test Technician (Radiographer) or to the Field Site NDE Supervisor (Radiographer) as appliable.

He is responsible for supervision of radiographic operations in accordance with NRC regulations as specified in the Radiation Safety Specification.

2.9 Radiographer (s) Assistant:

The Radiographer (s) Assistant performs Nondestructive Examination under the personal supervision of a Radiographer in accordance with NRC Regulations as specified in the Radiation Safety Specif-ication.,

QUALITY ASSURANCE PROGRAM 3.1 The program as described herein defines :

a.

The methods for review, approval and dis-tribution of quality assurance procedures for demonstration of conformance to NRC requirements.

b.

The procedures for handling, storage, and shipping to attain effective control and assurance to radiological safety and pack-age performance, c.

The methods for conducting leak tests,

radiation surveys, quarterly maintenance, and daily equipment checks to minimize the hazard of radiation exposure.

d.

The system for maintaining records suffi-cient to provide objective evidence that the required procedures, tests, inspections are performed, and the quality program is being adhered to.

3.2 Procedures

All radiation safety procedures snall ce written by the Radiation Protection Of-ficer and approved by the Manager of Nondestruc-tive Testing.

The Radiation Protection Officer shall insure that all Q. A.

functions are con-ducted in accordance with the latest revision to these procedures.

4.

DOCUMENT CONTROL 4.1 This procedure covers the marking, distribution and revisions of all radiation safety procedures i

and records.

4.2 Marking

All procedures shall be individually identified by a unique number and revision level.

All procedures shall be included in the Radiation Safety Manual.

Each manual shall be serialized sequentially and a record of the holder shall be maintained by the Radiation Protection Of ficer.

4.3 Distribution :

The Radiation Safety Manual includ-ing applicable procedures shall be distributed by the Radiation Protection Officer to all qualified radiographers, radiographer assistants, and trainees.

Personnel terminating employment shall return all Radiation Safety Manuals which have been assigned by the Radiation Protection Of ficer.

4.4 Procedure Precaration and Revision:

All procedures and revision thereto shall be written by the Rad-iation Protection Officer and approved by the 3

Manager of Nondestructive Testing.

4.5 Maintenance

Distribution of Revisions - A letter of transmittal including a copy of the revision I

shall be distributed by the Radiation Protection Officer to each. holder of a controlled copy of the Radiation Safety Manual and Quality Assurance Pro gram.

Each holder is responsible for maintaining and up dating his program, including the prompt insertion of all new and revised pages or sections when issued and destruction of all obsolete or superseded pages or sections.

Each holder shall return a completed copy of form QAP4.SA (Figure 1) which shall be sent with the transmittal letter.

HANDLING, STORAGE AND SHIPPING _

5.1 All handling, storage, and shipping of radio-active materials shall be performed by qualified radiography personnel.

This shall be performed in accordance with Radiation Safety Practice Specification.

No shipments shall be made until all provisions of this specification and the specification for shipping radioactive material have been accomplished.

INSPECTION, TEST, AND OPERATING STATUS 6.1 Inspections shall be performed in accordance with Radiation Safety Practice Specification.

6.2 Test:

Leak Testing shall be performed in accord-ance with the leak test specification prior to shipment and once every sir months.

Leak Testing shall be performed by the Radiation Protection Officer.

6.3 Operating Status:

The operating status of storage containers and of Isotope Projectors shall be labelled or tagged as applicable in accordance with Radiation Safety Practice Specification and Ship-ping Radioactive Material Specification.

Package identification shall be the responsibility of the Radiation Protection Officer or designated Qualified Radiographer.

6.4 Inspection and Maintenance of Isotope Projectors shall be performed in accordance with written pro-cedure and proper records and follow up maintained.

This inspection shall be performed by qualified radiographers.

The Radiation Protection Officer shall be responsible to insure that the functions, as required in the written procedures, are per-formed.

6.5 The operating status of storage containers and of Isotope Projectors shall be labelled and tagged in accordance with written procedures.

Package iden-tification shall be performed by the radiographers and the Radiation Protection Officer shall insure that it is performed as reqtlired.

AUDITS 8.1 Corporate Radiation Safety Program Audits shall be performed on a quarterly basis by the Manager, Corporate Quality Assurance, or his designee and shall be performed using a written check list.

8.2 Results of audits shall be documented for review by the Vice President, Research, Development and Engineering Division and Manager of Nondestructive Test ing.

8.3 No person directly connected with the implement-ation of the program shall be a member of the audit team.

8.4 Prompt follow up action including re-audit of deficient areas shall be performed.
