SUBJECT:

CLOSE0VT OF POTENTIAL GENERIC ISSUE ON IMPROPER FUSES INSTALLED IN CONTROL CIRCUITS AT BROWNS FERRY UNIT 2 As requested in your memorandum of July 19, 1983 to E. L. Jordan regarding incorrectly installed control circuit fuses of the backup control panel at Browns Ferry Unit 2, we have studied the potential generic aspects of the issue including the correspondence between Region II and the licensee during the fall of 1993. We have concluded that the misapplication of these fuses by on-site personnel was limited only to the Browns Ferry Units 1, 2, and 3 power plants.

Specifically, two Bussman MIC type fuses, which can provide a remote audible alarm, were found to be installed upside-down so that open fuse or circuit fault conditions in the affected control circuits would not be annunciated in the control room as desired. Also, Bussman MIN type fuses to be used in circuits which require local visual indication only, were incorretly installed in control circuits which require the alarm teature that the Bussman MIC type fuse provides.

To support our conclusion we have reviewed all fuse related LERs since 1980, and we have discussed the events surrounding the issue with the knowledgeable people from the Browns Ferry Station, the Region II office, and the manufacturer of the fuses, Bussman.

On the basis of these discussions, and review it was concluded that the problem was unique to the three power plants at the Browns Ferry Station.

In essence, it appears that the cause for the problem was directly related to inadequately trained on-site personnel who are responsible for checking and replacing fuses as necessary.

In this regard, the licensee has informed the NRC that they have established cnd have completed the training of certain on-site personnel that are res)onsible for checking and replacing fuses in safety systems at Browns Ferry Jnits 1, 2, and 3.

The training program was completed in January,1984.

Since the discovery of the fuse problem in July of 1983, no similar fuse problem has been reported from the Browns Ferry plants or from other nuclear power plants.
