NRC I & E BULLETIN 79-09 FAILURES OF GE TYPE AK-2 CIRCUlT BREAKER IN SAFETY RELATED SYSTEMS ENCLOSURES (1) JAFNPP Emergency Electrical Distribution System Diagram

Dear Mr. Grier:

The following is provided as the Power Authority's response to the subject bulletin:

1. General Electric AK-2A breakers are used in safety-related systems at the James A. FitzPatrick Nuclear Power Plant. There are four AK-2A-505 and fif teen AK-2A-25 breakers installed in the 600 volt emergency power systems.
2. The specific functions of the AK-2A breakers in the 600 vol ts emergency power systems are as follows:

MCC - Motor Control Center Because of the quantity of equipment powered from the motor control centers (MCC), it is not practical to list all of these loads in this response.

These loads include battery chargers, reactor protection system motor generztor set, LPCI independent power supply, instrumentation, and safety-related valva motor operators. A diagram of the JAFNPP emergency electric power system is included as Enclosure (1).

The undervoltage trip device for General Electric AK-2 breakers referenced in I & E Bulletin 79-09 is not utilized in the JAF electrical distribution system. Loss of off-site power is detected by under-voltage relays installed on each 4160 vol t bus that supplies power to the 600 vol t emergency bus.

2. Preventative maintenance of General Electric AK-2 breakers is detailed in JAFNPP Preventative Maintenance Procedure EP-008. This preventative maintenance procedure covers the following areas of low vol tages breaker main-tenance:

Section 3.2 Inspection 33 Cleaning

.4 Lubrication 3.5 Contact Resis tance, Gap, Sequence, and Wipe 3.6 Insulation Resistance Testing 3.7 Operating Mechanism Adjustment 3.8 Adjustment of Breaker Trip Devices 39 Breaker Insertion and Final Testing Preventative Maintenance Procedure EP-008 is based on the General Electric installation, operation, and maintenance manuals for Type AK-2A breakers and have been updated to reflect the recommendations of GE Service Advice Letter 9 3 175 (CPDD) to the extent that they apply to the JAF installation.

NRC I & E BULLETIN 79-09 Page Preventative maintenance of breakers at FitzPatrick is performed by Grade "C" electricians or higher. Grade "C" electricians and super-visory personnel in the Maintenance Department have received specific training in the maintenance of GE breakers and switchgear (including Type AK-2 breakers).

This training was provided by the General Electric Company.

3 The following schedule for preventati /e maintenance of GE Type AK-2A breakers installed ir. safety-related 600 volt electrical power systems will be adhered to:

a. EP-008, Sections 3 2, 3 3, 3.4, 3.8 and 3 9 will be performed on a once per refueling cycle basis,
b. EP-008, Sections 3 2 through 3.9 will be performed once every two refueling cycles.

Because of the concerns expressed in GE Service Advice Letter 9 3175 (CPD3), EP-008 in its entirety will be performed on all safety-related AK-2A breakers prior to or during the next refueling outage.
