Subj ect: Noncompliance at Enrico Fermi Unit II Construction Site This letter responds to the infraction contained in your IE Report No. 50-341/79-08. This inspection of Enrico Fermi Unit II Site Con-struction activities was performed by Mr. E. W. K. Lee on March 27-30, 1979.

Only the cited infraction mentioned in your report is discussed in this reply, as required by Section 2.201 of the NRC 's " Rules of Practice",

Part 2, Title 10, Code of Federal Regulations. The unresolved items have been acted on. We will be prepared to report in detail on our progress and corrective action on these matters to your inspectors on their next visit.

The enclosed response is arranged to correspond to the sequence of items cited in the body of your report. The finding numbers from the report are referenced as well as the section numbers.

  ,             Response to NRC Report # 50-341/79-08 Statement of Infraction 79-08-01 Appendix A " Notice of Violation" Paragraph 4.a Infraction: Contrary to 10CFR50, Appendix B, Criterion V, repair of a linear indication by grinding on Spool No. G33-3096-3 adjacent to Weld No. OW3 was not accomplished in accord-ance with documented instructions, procedurcs, or draw-ings. The wall thickness af ter grinding was not ultra-sonically tested nor was the ground-out area unifccmly blended into the surrounding surface.

Corrective Action Taken ari Results Achieved Upon questioning all parties involved, it has been established that a linear indication was identified in the base metal adjacent to Weld No. OW3 wh2n liquid penetrant examination was performed on the weld. Also, in the same area, there was evidence of arc strikes and weld spatter which were not noted on DDR Report #188. In repairing the linear indi-cation, the are strike and weld spatter were repaired by the same craf ts-man. He was not aware that DDR #188 did not include the removal of the are strikes and weld spatter. An ultrasonic check was not completed to determine the remaining wall thickness before closing DDR #188. DDR #405 was initiated on 4/4/79. The disposition of the DDR is to blend ground area and determine if wall thickness is violated by ultra-aonic examination. If it has been determined that the wall thickness has been violated, an applicable Rl-WPCS will be prepared and the area will be welded in accordance with WPS-108. Ccrrective Action to be Taken to Avoid Further Noncompliance Per DDR #405, a written reprimand will be issued to the general foreman for the unauthorized grinding. Additional training is to be given to the individuals f nvolved in the removal of are strikes. WB-C-110 will be re-vised to specify ultrasonic examination.
