Gibbs and Hill Incorporated Docket No. 99900524/79-01 NOTICE OF DEVIATION Based on the results of a NRC inspection conducted on March 26-30, 1979, it appearce that certain of your activities were not conducted in full compliance with flRC requirements as indicated below:
10 CFR 50, Appendix B, Criterion III (Desigr. Control) states in part,
" Measures shall be established to assure that applicable regulatory requirements and the design basis... to which this appendix applies are correctly translated in specifications, drawings, procedures and instructions."

Implementing procedure DC-2, Revision 3 (Design Description Procedure) states in part, "The codes and standards applicable to work covered by the Design Descriptions (Technical Descriptions) are listed by publication specifying issue date and applicable section or sections."
Contrary to the above, while specifying the publication and applicable section or sections (e.g. ASME Section III) the Design Descriptions (Technical Descriptions) listed below do not specify the issue date of the applicable codes and standards:

(2) 2323-TD-0241, Revision 1 Demineralized and Reactor i

(3) 2323-TD-0215, Revision 1 Diesel Generator Fuel Oil Storage and Transfer System B.
Implementing procedure DC-10, Revision 2 and Revision 3 both state in part, "After completion of his work, the Design Reviewer attaches a signed Design Review Record Form... and returns this along with a completely filled in checklist to the QA Engineer."
Contrary to the above, after completien of design review on LOCA Analysis (Filing Code No. 2323-NC-AA-05), a checklist was not completed by the Design Reviewer for submittal to the QA Engineer.
