Westinghouse Electric Corporation Docket No. 99900047/79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on January 8-12, 1979, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:

A.

Criterion V of Appendix B to 10 CFR 50 states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings...."

Item number 2 of Technical Quality Assurance Instruction 1.5.2, Revision D, states in part, "... will maintain a log of welders to whom electrodes are issued and from whom electrodes are received."

Contrary to the above requirements, it was observed that the weld log for the welding of a shipping cover on shop order 2973 could not account for 110 electrodes issued to three (3) welders on the previous shift.

B.

Criterion V of Appendix B to 10 CFR 50 states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appmpriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings...."

In addition, paragraph 2 of Part 1, to process Specification 83050RM states, " Electrodes shall not be exposed to ambient temperature and humidity for longer than two (2) hours without being rebaked in the above cycle, providing identity is maintained."

Contrary to the above requir er,s, it was observed at 10:10 am that three (3) welders were weiding on pressurizer shell weld 2161BL to 2161DL with 9018 we'id rod checked out at the start of shift (7:30 am) and not rebaked during this period.

C.

. In addition, Part 1 of Process Specification 82121, Revision 2, requires welding to be perfonned in the amperage range between 320 to 375.

Contrary to the above requirements, it was observed that repair welding of H seam of pressurizer on shop order 2551 was being perfonned using the above referenced process specification, at 295 amps.
