SECTION I STATD4ENT OF RESPONSIBILITY Af[D AUT10RITY The management of O'Neill Tank Company establishes and implements this QA Program in compliance with the United States Nuclear Regulatory Conraission 10CFR71 Requirements.
Training for all Qk functions, prior to engagement in these functions, is required according to written procedures. QA Program revisions will be made according to written procedures with management approval. The QA Program will ensure that all defined QC procedures, en6 neering 1
procedures, and specific provisions of the package design approval are satisfied. The QA Program will emphasize control of the characteristics of the package which are critical to safety.
The Radiation Safety Officer shall assure that all radioactive material shipping packages are designed and manufactured under a OA Program approved by the Nuclear Regulatory Commission for all packages designed or fabricated after July 1,1978. The Responsibility for design and fabrication of radioactive material shipping packages used by O'Neill Tank Company rests with the supplier of such containers. At this time, the only supplier utilized is listed below:

(Attached The Radiation Safety Officer shall be responsible for overall adminis-tration of the program, training and certificatidn, document control, and auditing. Radiographers are responsible for handling, storing, shipping, inspection, testing, operating status and record-keeping.

SECTION II RECORDS The maintenance of all records, sdch as shipping papers and receipts, including any references and drawings, inspections, tests, operating logs, audit results, personnel training and qualifications, records of shipments, description of equipment and written procedures shall be carried out by the secretary of the Quality Control / Radiography office in Great Bend, Kansas, with the assistance of the Quality Con-trol Manager and the Radiation Safety Officer. Such records will be maintained in accordance with written procedure and shall be identi-fiable and retrievable. A list of such records with their storage lo-i cation shall be maintained.
The Radiation Safeby Officer shall insure that all QA functions are conducted in accordance with the latest applicable changes to these documents.
SECTION III HANDLING, S'IORAGE AND SHIPPI?G Written safety procedures concerning the handling, storage and ship-ping of packages for certain special fom radioactive material, such as that used by this licensee, will be followed. Shipments will not be made unless all tests, certifications, acceptances, and final in-spections have been completed. Such activities shall be governed by OTCI procedure, " Movement of Radiographic Devices".

" Procedure For Using Tech-Ops 414 or 650 Source Changer". (Exhibit E)
Only Radiography personnel shall perfom the critical handling, storage and shipping operations involving radioactive packages and devices.
All handling of packages of>: radioactive material shall be docu-mented through use of Radiography Production Cards. (Exhibit C) 2259'110
SECTION IV INSPECTION, TESTS AND OPERATING STATUS Inspection, tests and operating status of packages for certain special form radioactive material will be indicated and controlled by OTCI written procedures. For operation of radiographic equipment, radiographers and absistants shall follow " Operating Procedure For Remote Control Radiographic Devices",(Exhibit F). Inspection and Maintenance operations shall be governed by " Inspection and Mainten-ance For Remote Control Radiographic Dev3 :es", (Exhibit G).
Status shall be documented by entry on Radiography Production Cards.
Status of any nonconfoming parts or packages shall be positively maintained, and corrective action taken accordingly.
Source leak tests shall be performed at least once every sbt months for a given source. OTCI " Procedure For Leak Testing" shall be followed. (Exhibit H)
Only Radiographic personnel shall perfom these regulatory required inspections and tests, and the above procedures shall be strictly adhered to. It is the responsibility of the Radiation Safety Officer to insure that these functions are perfomed.

SECTIDN V D4ERGENCY PROCEDURES O'Neill Tank Company General Radiation Emergency Procedures, (Exhibit B), shall be followed in the event of an accident or loss of a r3ource or a radiographic device, where the possibility that radiation danger may exist, however minute that possibility may be.

During normal working hours, this call will go to the Radiological Health Section, State of Kansas. After hours the operator will locate the appropriate person to return the call, The caller should remain at his telephone until called back. For further, more detailed information, refer to OTCI Radiation Dnergency Procedures, (Exhibit B).
These procedu es are posted on the doors to the radiography supply rooms of each shop, as well as the Radiography Department in the Great Bend office.
SECTION VI AUDITS Established schedules of audits of the Quality Assurance Program will be performed using written checklists. Results of audits will be maintained and reported to management. Audit reports will be evaluated and defecient areas corrected. The audits will be dependent on the safety significance of the activity being audited, but each activity will be audited at least once per year.
Audit reports will be maintained as part of the quality assurance records. Members of the audit team shall have no responsibility in the activity being audited, however a radiographer should be present for interpretations and safety consideration.
