Attachment L Description of Quality Assuunce Program for Type B QuantiLJ Shipments of Radioactive Material 1.

Organi:ation

a. blisconsin Industrial Testing, Inc. putsuant to 10 CFR 71.12 and 10 CFR 71.51 (a) is responsible for a QA Program for Type B quantity shipments and transportation of radioactive material. FabrienPan and design of Type 8 packages are not included.
b. The organisatinnat sttueture in Attachment H is applicable.
1) The Rndintinn Safe.ty Officer is responsible for administration of he program, btaining of peMonnel, procurement af manufactater*s certificat. ions, mintaining pnehnnLng and shipping documents, retention of records and program artditA.

Radiographen are responsible for handling, storage, shipping and transporting radioactive packages in accordance salth the procedures in Attachment E: and key are responsible fo.t inspection, surveying and maintenance reports in accordance salth the procedures h Attachment E.

QualLty Assurance Program

a. Radiographers are trained in their raponsibiti. ties above according to Attachment F.
b. The QA program sciLL be viable,.tequiring mnagement approval, and be program encompasses specific requirements s. tat.ed in the package approval for use, and general radiation safety and shipping requi.tements.
c. Mannfacturer's certifications scill be requested and maintained on all NRC Type 8 packages manufactured and procured after January 1,1979.

Document Control

a. Packaging, shipping, inspection and handling insttuctions fram ue manufactureu scill be mintained for each model Type 8 package,
b. The RSO shallinsure hat the shipment and L1ansportation of Type 8 packages is conducted in accordance teith the above documents.

4.

Handling, Shipping, Transporation and Storage

a. Radiography peuonnel scill handle, ship, Ltansport, and st. ore radioactive packages in accordance saith the proceda,tes in Attachment E.
b. Radiography personnel sciLL determine the completeness of package inspestions and presence of manufacturer's certificates beforeshipment.

5. Inspections, Surveys and Maintenance Reports
a. Inspections, surveys and cnin.tenance reponts on Type B packages witt be made in accordance with Attachment E procedures.
b. Quarterly inspection and maintenance of Type B packages wilt. be perfarned by the RSO as ou.tLined in Attachment H.
6. Quality Assurance Records
a. UtiUratinn togs, shipment records, packages surveys, and quarterly inspection records will be maintained as spt.ctfLed in Attachments E cud H.
b. Training records are maintained in accordance with Attachment H.
c. Equipment and package, procedures and inst. ructions are meintained by the Radiation Safe 2.y 0fficer.
7. Audits
a. Quarterly audits of the entire edininn safety program of Wisconsin Industrial Testing, Inc. is performed in accordance with Attachment G.
b. These quarterly audits include the necessary checks on the QA program as outlined in Attachment H.
