Dear Mr. Grier:
This telegran is to confirm the telecun between Mr. Don Haverkamp, Region I, NRC and J, L. Seelinger, Unit 1 Superintendent at 10do on June 19. 1979.
During inspection of tne pnospnor bronze operating rods on relays in ene Engineered Safeguards Actuation Systera, relay 62x28/RCIA failed in the energlZed position. The relay did not crop out wnen the coll was de-energized.
Tne cause for the failure may oc wear on tne operating rod caused by vibration of tne relay wnile energized.
ints problein is potentially generic and tnerefore reportaole in accoroance witn Tecnnical Specification 6.9.2A(9).
ine tailed relay is a logic relay tnat is part of the two of Enree coincidence logic for Uecay Heat Closed Cooling Pu.ap A and Nuclear Service Closed Cooling Puitp A.
Inese two components are un loading sequence block 3.
Tne failed relay is a GTE Sylvania Type PM, Bulletin 7305 Catalog 5U12-11.
Corrective action is Deing investigated.
