APPENDIX A NOTICE OF VIOLATION Vermont Yankee Nuclear Power Corporation Docket No. 50-271 Based on the results of an NRC inspection conducted on January 22-25, 1979, it appears that certain of your activities were not conducted in full compliance with NRC regulations and the conditions of your NRC Facility License No. DPR-28, as indicated below.

Item A is categorized as an Infraction; Item B as a Deficiency.

A.

Section 6.5.A of the Technical Specifications requires, in part, that procedures written to implement the Emergency Plan La followed.

Procedure OP-3012, Revision 1, " Emergency Plan Training," written pursuant to the above requirement, requires, in part, that for Emergency Coordinatcrs to be qualified initially or to maintain qualifications, these personnel must I.

ticipate annually in at least two classroom exercises evaluating various meteorological conditions and calculation of offsite airborne activity.

Contrary to the above, as of January 25, 1979, four of the nine personnel designated as Emergency Coordinators did not participate in at least two classroom exercises, as defined above, since November 1977.

Sections 6.5. A and 6.5.B of the Technical Specifications require, in part, that procedures be established and followed relative to emergency conditions and radiation control, respectively.

Proce-dure DP-4559, Revision 1, written pursuant to the above require-ments, specifies, in part, that personnel dosimeters be calibrated at six month intervals.

Contrary to the above requirements, as of January 25, 1979, personnel dosimeters, including Nos. 504004, 50406, 12987, and 13053, from the emergency equipment kits were not calibrated since February, 1978, exceeding their required calibration intervals.
