As a result of our review, we find that additional information, as discussed with you during a telephone call on October 27, 1978, is needed on the following:
Items A and B Previous commitments i.e., indoctrination and training of personnel, made for steps to prevent recurrence on similar past findings, have not eliminated errors in MRS utilization relative to your QA Program Commitments.
Please describe what additional actions are planned to assure the effectivity of your current proposed indoctrination and training of personnel'.
Please provide this additional information within twenty (20) days in order that we may complete our review in a timely manner.
