50-382/78-14 Appendix A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on November 14-17, 1978, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Construction Permit No.
CPPR-103 as indicated below:
Failure to Establish Surveillance Inspection Program 10 CFR Part 50, Appendix B, Criterion X requires that a program for inspection of activities affecting quality shall be established to verify conformance with documented procedures for accomplishing safety related activities.
Tompkins-Beckwith Quality Assurance manual, paragraphs 11.4.1.1 and 11.4.7.1 specify that a Quality Control Program will be performed which will include recording of the QC surveillance inspection of welder's conformance to weld procedure requirements including gas flow, volts, amps, etc.
Contrary to the above:
Tompkins-Beckwith (T-B), who are involved in the welding of safety related piping, have not established a documented program for per-forming surveillance inspection of welder's conformance to weld procedure requirements.
This finding was confirmed during dis-cussion with T-B QA personnel.
This is an infraction.
