Combustion Engineering, Inc.

Docket No. 99900401/78-03 NOTICE OF DEVIATION Based on the results of a NRC inspection conducted on September 26-28, 1978, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:

A.

10 CFR 50, Appendix B, Criterion III, Design Control, requires

" verifying or checking the adequacy of design."

Implementing procedure PE QA No. 001 which requires verification of Project Specifications, states in Section 6.3.2:

"At times it may be necessary to use or transmit copies or results of documents that have not received an independent review. The cover page of the document will be clearly marked "NOT REVIEWED" (or equivalent wording).

For followup purposes, the transmittal sheet for unreviewed documents will indicate when these documents will be reviewed."

Contrary to the above, Project Specification 14273-PE-410, Revision 1, dated February 26, 1976, for Safeguard Pumps (Low Pressure Safety Injection-LPSI; High Pressure Safety Injection-HPSI; and Containment Spray-CS) is marked NOT VERIFIED, but transmittal sheet does not indicate when it will be verified. Pumps have been manufactured, tested and some parts shipped.

10 CFR 50, Appendix B, Criterion VI, Document Control, requires that

" measures shall assure that documents, including changes, are reviewed for adequacy and approved for release by authorized personnel.

Changes shall be reviewed and approved by the same organization that performed the original review...."

Implementing procedure QADP 6.0, paragraph 6.0.7, Preparation and Handling of P.O. (Purchase Order) Supplements, states in part:

" Preparation of a P.O. Supplement shall follow the same sequence as the original ordar requisition and the same review and approval is required."

Contrary to the above, P.O. Supple.aent 3B to P.O. 9403833 to CE-KSB for Reactor Coolant Pump did not receive the same review and approval as the original, although quality Iffecting changes were invoked.
