Dear Mr. Keppler:
This refers to the Notice of Violation dated November 29, 1978 and received at Clarke College on December 6, 1978.
Please note that our License number is SNM-779, not SNM-799 as shown on the letter. We have taken the f ollow ing steps to correct the items of non-compliance.
Item 1 - Calibration of radiation protection equipment With the exception of the Nuclear Chicago #2526 Cutie Pie Survey Meter, all items for radiation protection listed in the original application (March 4, 1965, #6) are in working order and have been calibrated according to manufacturers instruction this month.
The quarterly celibration will be the respcasibility of Sister Mary Louise Caffery, Ph.D., Chairman of the Chemistry / Physical Science
. Department. A log book will be kept with this record and the leak test results.
Sister Caffery has contacted.Searle (formerly Muclear-Chicago) regarding the Cutie Pie Survey Meter. However, it is unlikely that parts or repair service will be available since the instrument is almost 15 years old. We have no plans to replace it since new ones cost over $500 and we so rarely use the licensed material.
We believe the other equipment available will be sufficient for safe use of the material.
If this is not satisfactory please notify me.
Item 2 - Leak Test of Plutonium Source Sister Caffery has assumed responsibility for performing the leak tests effective immediately.
The tests will be performed at the beginning of each academic term, i.e.,
in January and early August.
Sister Caf fery has read the pertinent material and understands the process and the calculations necessary to record the evaluation in units of microcuries.

A Mr. James G. Keppler Page two December 12, 1978 Item 3 - Supervision of Material On November 7, 1978 we requested an amendment to condition 12 of our license. We received the notice that the amendment was granted in a letter from the License Material Branch of the U.S. Nuclear Regulatory Commission on November 27, 1978.
Condition 12 now reads " Licensed material shall be used by, or under the supervision of, Sister Mary Louise Caffery, Ph.D."
Item 4 - Posting of License and Amendments A notice has been posted in two places that the license and all materials pertaining to the license are available for inspection in the office of Sister Mt y L. Caffery (314 CBH). These are:
Rn 6 Catherine Byrne Hall - Location of Neutron Howitzer Rm 332 Catherine Byrne Hall - Radioisotope Preparation Lab Item 5 - Notice to Employees Copies of Form NRC-3, " Notice to Employees" have been posted in the same locations as above.
The non-compliance was due primarily to a change in personnel at the College.
Sister Mary L. Caffery who is now supervisor of the material is a tenured faculty member and has accepted responsibility-for the correct management of the program and compliance with the license requirements.
She intends to organize the file containing material pertaining to the program during the January interum.
By January 15, 1979 our files will be in good order and the records of calibration and leak tests will be properly maintained.
This will facilitate transfer of supervision to another person in the event that Sister Caffery is away from the campus for any length of time.
Thank you for sending copies of 10 CFR Parts 19, 20 and 70 which we requested.
I trust that these actions have brought us into full compliance with the license requirements.
