I Appendix A NOTICE OF VIOLATION Quality Testing, Inc.
License No. 34-17799-01 Based on the inspection conducted on March 18, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Items 1 through 3 are infractions, and item 4 is a

deficiency.
1.
10 CFR 34.24 requires that each radiation l survey instrument shall be calibrated at intervals not to exceed three months.
Contrary to this requirement, a Victoreen Model 492 survey meter, Serial No. 650, was not calibrated between February 23, 1979 and December 13, 1979. A review of records showed this survey instru-ment was used on nineteen occasions during the period September 14, 1979 through November 30, 1979.

10 CFR 34.26 requires that each licensee shall conduct a quarterly physical inventory to account for all sealed sources received and possessed under his licenre.
Contrary to this requirement, no inventory was made between July 9, 1979 and December 15, 1979, a period in excess of three months.

License Condition No. 16 states, in part, the licensee shall possess and use licensed material in accordance with statements, representa-tions, and procedures contained in the letter dated January 23, 1978.
One of these procedures states it is the responsibility of the RSO to perform an inspection of the equipment at intervals not to exceed three months.
Contrary to this requirment, no inspection was made between July 9, 1979 and December 15, 1979, a period in excess of three months.
4.
10 CFR 20.408 states, in part, that when an individual who porsesses or uses byproduct material for purposes of radiography terminates employment the licensee shall furnish to NRC a report of the individ-ual's exposure to radiation incurred during the period of employment.
Contrary to this requirement, no report was submitted for three individuals who terminated employment during 1979.
