Dear Mr. Grier:

On November 2,1979, the Nuclear Regulatory Comission IE Region III was informed by Union Electric of deficiencies in three (3) pre-assembly piping formations at Callaway site. These deficiencies were reported by telecon to Ms. Jolene DuBry of Region III pursuant to the requirements of 10CFR50.55(e). Since that date and, based upon examinations of other formations involving both Callaway and Wolf Creek Units, these deficiencies are considered to be generic in nature and therefore applicable to all SNUPPS units.

Enclosed with this letter is a report of the referenced preassembly formations. This report describes the deficiencies in preassembly fabrication, inspection, and NDE; provides a brief chronology of related events; summarizes potential safety implications resulting from these deficiencies and outlines follow-up corrective acticns that have been initiated. Follow-up actions to rework on site and, where necessary, repair Gulf & Western furnished preassembly formations are being initiated by the jobsite constructors and will be processed in accordance with existing site rework procedures and controls. No further work by Gulf & Western (G&W) on safety-related preassembly formations for other SNUPPS Units; i.e. Sterling or Callaway No. 2, will be authorized by SNUPPS without demonstration of substantial improvements on the part of GLW in welding, NDE, QA and management controls, f 2I 90 02146 M

The deficiencies described in the enclosed report have also indicated need for improvements in Bechtel and Code Authorized Nuclear Inspector (ANI) surveillance and inspection functions. Actions to strengthen the Bechtel effort have been initiated. Steps to re-examine the ANI function are being established and will be pursued separately.
