Please note that our eventual approval of the Quality Assurance Program Plan and its application to the one-time foil transportation does not constitute approval of the details provided in Attachment I of the submittal as these details are beyond the scope of our review.

Request for Additional Information 1.
Section 1.0 Organization must correlate with organization chart I
(Attachment 2).
It appears that this can be done satisfactorily by:
a) Showing the Radiation Safety Offices and his reporting relationship on the chart.
If the Chief, Radiation Health Protection is also the Radiation Safety Offices, this can be shown by showing both titles in the same block.
b) Showing the Radiographers and their reporting relationship on the chart.
Please revise the submittal to correlate the text and the chart.
Note that the names of individuals are not required, i.e., position titles above are acceptable.
2.
In Section 4.0 Handling, Storage, and Shipping indicate who, by position title, performs the handling, storage, and shipping operations.
3.
In Section 5.0 Inspection, Test, and Operating Status indicate who, by position title, performs the inspections and tests and who assures that these functions are performed.
4.
In Section 6.0 Quality Assurance Records provide statements that; a) The records will include descriptions of equipment and written proce-dures.
b) The records will be maintained per written procedures.
c) The records will be identified and retrievable.
d) Identify who will maintain a list of the records and their storage location.
