Action on Previous Survey Follow-up Items The following items were identified during the survey 99900601/77-02 as follow-up items.

Although Hartford Steam Boiler was notified that these items were for the USNRC Review Committee and required no response or action by HSB/ SIS, the inspector reviewed the following corrective actions by HSB/ SIS related to the four (4) items:

1.

Records of previous experience related to the requirements of ANSI N626.0, Section 4.1, were not being maintained by the Regional' Manager per the HSB/ SIS Manual, but were being maintained in another area of the Houston office.

The inspector verified that these records were now being m-intained in the personnel files of the Regional Manager.

2.

Detailed procedures were not available that identify the mechanism that the ANI should use to report code violations. The inspector verified that Section 7400, Inspection Duties, of the HSB/ SIS Manual, now contains this requirement in item 3.6.1.

Form 939, SIS Record for Monitoring QA/QC Programs, will be used for docu-menting these code violations as ncted in instruction 7619.

3.

Procedures were not available that would identify the types of records that should be maintained in each supplier file. The inspector reviewed an instruction prepared by the Regional Manager, '~

dated July 20, 1978, that identifies those records that are to be maintained in each supplier file in the Regional Office.

4.

Procedures that describe the mechanism for referring technical questions to the Regional Office were not available.

reviewed Forms 932, SIS Report, and 937D, SIS Daily Inspection Record, that document technical questions referred to the Regional Office by the ANI.

Instructions 7613 and 7615 refer to the method of completing the forms.

Authorized Inspection Agency (AIA) Organization Nuclear ASME Code Inspection activities in the Hartford. Steam Boiler Inspection and Insurance Company (HSB) are the principal responsibility af the Special Inspection Service Division (SIS). Regional offices of SIS are established at various locations throughout the United States.

In this region, the Houston branch office of HSB pro-vides administrative services to the SIS Regional Manager and his staff.

D.

Qualifications 1.

AIA The AIA must meet several requirements imposed by the National Board and the various jurisdictions in order to perform inspection se rvices. They first must be autharized by the state insurance commission, etc., to sell B&PV liability insurance within their jurisdiction. However, they are not required to have an active insur uwe omtract with the supplier to whom they provide inspec-tion service!.. HS3 representatives stated that in some cases they provide both insurance and inspection services while in others they provide inspection services only. They must also have a staff of inspectors commissioned by both the jurisdiction (state, city, county, or province) in which the inspection service is to be performed and the National Board.

In discussions with HSB/ SIS management the inspector determined that HSB is licensed to operate in all of the fifty states.

Inspection Personnel The Houston office of HSB-SIS has an inspection staff of twenty-five (25) inspection personnel, of which there are sixteen (16)

ANI's, five (5) inspection specialists, and four (4) supervisors.

Of these inspection personnel there is a further breakdown into two (2) ANI (Inservice), one (1) ANIS (Inservice), two (2) ANI (Concrete), and one (1) ANIS (Concrete).

4 Oklahoma and Texas are the two states serviced by the Houston Regional Office.

The sixteen ANI's are located in various cities in Texas and Oklahoma and receive technical and administrative direction out of the Houston office of HSB. Five of these ANI's are presently providing contract inspection services at the Co=anche Peak (Texas Utilities Services Company) and at the South Texas Project (Houaton Lighting and Power Company).

Four ANIS's provide technical assistance, training programs, and review reports and maintain records for the ANI's in the field.

The minimum qualification requirements established by FSB for personnel hired as inspectors are that they have sufficient experience to satisfy the National Board requirements for com-mission as an Authorized Inspector (AI). The National Board requires a minimum of three (3) years experience in boiler shops, installation, operation, or inspection prior to taking the juris-dictional and National Board examination.

These examinations are prepared by the National Board and given by the jurisdiction in which the initial application is made. An applicant for a National Board Commission must hold a Commission or certificate of competency from a jurisdiction.

Some jurisdictions require an additional examination for a reciprocal commission e.g. New York requires that the inspector appear before the Chief Boiler Inspec-tor for an: oral examination on State requirements 1[.

There do not appear to be any additional requirements imposed by any juris-diction with respect to the special endorsement for the ANI, ANII, etc.

Their qualifications are established by the National Board.

HSB-SIS requires that a trainee must gain one year's experience as a qualified authorized inspector prior to participation in the Authorized Nuclear Inspector Training Program. The first assign-ment of the trainee requires two to four weeks on job training in a nuclear shop under the direct supervision of an ANI. The trainee is then enrolled in several of the

""B (Hartford) training courses. These courses are listed in order:

ANI School (one week),

Radi tion Monitoring and Health Physics course (two days); this is followed by two to four weeks of nuclear in-shop evaluation by the principal ANI. The trainee then attends the National Board ANI training school (two weeks) prior to being examined and commissioned as an ANI by the National Board.

5 The inspector reviewed the experience, qualifications, and training records of eight (S) Authorized Nuclear Inspectors that are assigned to the Houston Regional office of USB-SIS.

Section 3.4.3.4. of Chapter 7200 of the HBS-SIS Manual requires that " Evidence of experience shall be maintained by the Regional Manager." The inspector was able to obtain this information from pre-employment applications which are on file in the Regional Manager's Office. The experience, qualifications, and training of the individuals' records that were reviewed were consistent with the requirement of ANSI 626.0.

AIA Performance of Duties 1.

Policies, Procedures, and Instructions HSB-SIS polit ies, procedures, and instructions are contained in the following manual:

Inspection Handbook-Special Inspection Services Section. This manual details the administrative functions of the home office, regional offices, and branch offices.

This manual also includes procedures for the training and development of SIS person-nel and the inspection responsibilities and duties of both nuclear and non-nuclear inspectors.

Training Training programs related to Authorized Inupector, Authorized Nuclear Inspector, Authorized Nuclear Inspector Supervisor, Assistant Regional Manager, Authorized Nuclear Inservice Inspector, Authorized Nuclear Inservice Inspector Supe rvisor, Authorized Nuclear Inspector Concrete, and Authorized luclear Inspector Concrete Supervisor are described in Chapter 7300 of t he manual identified abeve.

The Regional Manager maintains records of On Job Training and s*:tendance at company sponsored and National Board training programs for each ANI.

Auditing of Inspection Personnel The Regional Manager is required to audit each of the ANIS at nis shop at least semiannually. The Houston Regional Manager has begun implementation of this audit function. The Regional Manager is in turn audited by the Assistant Manager SIS (USA)

(Hartford) on an annual basis. The inspector reviewed the audit reports for 1978 and 1977.

The 1979 audit was being conducted at the time of this inspection.

6 was conducted somewhat along the format of ANSI N45.2.12, i. e.,

notification, preaudit conference, reporting, response, corrective action, and verification of corrective action. The checklist was made up from the requirements in the manual of the Regional Office Functions-Chapter 7200.

Follow-up Items Inspector appraisals had not been conducted annually as a.

required by the HSB-SIS Manual.

Nuclear shep mdits had not been performed semi-annually as required by ANSI N626.0.

F.

Contracts Serviced The Houston Regional Of fice of HSB/ SIS presently is servicing contracts at twenty-three (23) nuclear component suppliers and two (2) nuclear utility projects.

G.

. Exit Meetin_g An exit meeting was held with management representatives on August 29, 1979. Those individuals in attendance at the exit meeting are indicated in paragraph A. above.

The inspector discussed the details of the survey and the items identified in this report as follow-up items. The inspector stressed that these items were for the USNRC Review Committee and required no response or action by HSB-SIS.
