A re-examination of Grinnell piping radiography by licensee's QA following acceptance and shipment of piping, revealed deficient radiography on 68 welds out of 523. The radiography on an addi-tional 170 welds was deficient if allowance is not made for Code Case 72, which has not been permitted by licensee's specifications.

Action by Licensee:

5 The 68 velds are being re-radiographed and the decision not to incorporate Code Case 72 is being reviewed.

The four inspectors involved have been removed from all the li-censee's work, and other work inspected by these four in spectors will be audited for acceptability.

This item will be reviewed during the next inspection.
