This situation related to Ccndensate Storage Tank 1E Diaphrag2.
Descri; tion of Incident During ec plete draininc of Condensata Ctorage T1nk lE to the condenser hotvell, the diaphracn was sucked over the euction screen cutting off flow.
The condenser vacuun acting on the diaphragn ecllapsed the cuction screen ':hich resulted in the diaphragm being punctured in several places.
Corrective Action Taken The tank va; optned to inspect the sucticn screen which was fcund to be collapsed and a new cereen was designed by the Architect Engineer which will prevent the diaphra.en fron biccking the suction and also prevent the diaphragn from beinc punctured.
The diaphrag= vaa repaired and the tani put back in service.
The new designed suction screen vill also be installed in Condensate Starace Tank 1A to preclude this tyce of incident frcn occurring.
As a further precaution, an operating limit has been i posed by Metropolitan Elicon Cc pany to preclude the diaphragm frc cein6 in contact with the suction screen.
