Operating License DPR-50 Docket No. 50~289 The following is a report of a condition which was originally considered to represent an abnormal occurrence but which, upon review, has been deter-mined to not represent an abnormal occurrence as defined by the Technical Specifications for our Three Mile Island Nuclear Station Unit 1 (see the discussion under Item 8 below). It should be noted, however, that we have determined this condition to be reportable in accordance with Section 6.7.2.a.4. of the Technical Specifications, and that this report is submit ted to satisfy the reporting requirements thereof.

Identification of Occurrence

Title:

Fire on the Exhaust Manifold of the EG-Y-1B Emergency Diesel Generator Type: See opening statement.

(5) Conditions Prior to Occurrence: The reactor was at steady-state power with major plant parameters as follows:

Power: Co re : 2535 FM (Gross)

Elec: 870 MW 4n 204 6

RC Flow: 140 x 10 lbs/hr

.. k 1 RC Pressure: 2155 psig RC Temp: 579 F PR2R Level: 240 in.

650 F (6) Description of Occurrence During routine surveillance operations of Emergency Diesel Generator EG-Y-1B, in preparation for placing the lA Emergency Diesel Generator out of service for maintenance, a small fire occurred in the lagging around the diesel engine exhaust manifold adjacent to the turbocharger exha ust gas inlet.

The fire was extinguished, and as a precautionary measure to ensure that it was not damaged, the engine was shut down and inspected. As this immediate inspection of the engine revealed no apparent damage, the engine was restarted, the operational surveillance procedure was completed satisfactorily, and the 1B Diesel Generator was returned to service.

(7) Apparent Cause of Occurrence The fire resulted from a small amount of oil leakage, from an engine inspection cover plate joint, which soaked into the exhaust manifold lagging and was subsequently ignited by the hot engine exhaust manifold.

Furthermore, it has been determined that the apparent cause of the fire was design, in that had oil deflectors been designed and installed to prevent oil from coming into contact with exhaust manifolds, the fire would not have happened.

(8) Analysis of Occurrence It has been determined that fire did not constitute an abnormal occurrence and did not, therefore, constitute a threat to the health and safety of the public in that a.

the diesel generator was available for ESAS use throughout the course of the condition, and b.

even if the fire had been undetected, the amount of leaked oil was so small that the fire would have simply burned it-self out, and would not have effected the operational capability of the diesel generator.

(9) Corrective Action Immediate corrective actions were taken as described above to extinguish the fire, to ensure the diesel engine was not effected, and to return it to service. In addition, all accumulated oil was wiped from the engine, the bolts on the suspect cover plate were checked to ensdre they

were tight, and temporary oil deflectors were fabricated and installed to keep oil off the exhaust manifold.

Ar a meeting held shortly af ter the condition, the Plant Operations Review Committee (PORC) met, approved the immediate corrective actions, and in addition, recom:nended to the Station Superintendent that the following long-term preventative actions be taken:

a.

contact the manufacturer in an attempt to obtain permanent oil deflector pans to keep oil off of exhaust manifolds, and b.

evaluate a suggestion from the manufacturer to seal possible oil leaks with Room Temperature Vulcanization (RTV).

The Station Superintendent concurred with PORC 's findings and re-commendations, and has taken steps to ensure implementation of the long term preventative actions.
