Response

Incidents involving in plant perscnnel errors result in the following trainin6:

A.

Perscnnel involved are counseled depending en the circumstances surrounding the event.

B.

The incident is reviewed in the requalification program.

C.

In sete instances, event reports are written and circulated to personnel who need to benefit in the shnrt ter= frem the personnel error experience. Clearing House Documents, Operating Experience Reports and Event Reports are reviewed for applicability to TMI and discussed as part of the Requalification Program.

tatively assess the performance of the operating and support staff in such areas as adherence to operating procedures, use of systems checklists, and i=plementation of cc=ponent and system tagouts. This should include review of the degree to which operating procedures, tagout procedures, and check-lists require signoff, i.e. signature or initials to verify proper completion and to identify the respcnsible personnel.

Rescense:

Virtually all operating procedures and checklists require signoff by the individual who perfor=ed the evolution plus review and signature by an independent Reactor Operator or Senior Reactor Operator. Most of the subject procedures require initials next to each step to assure satisfactory completion.

The quality of operations is controlled through the use of these procedures which are reviewed by the Plant Operations Review Cc==ittee and approved by the Station Superintendent prior to their use, in accordance with the Technical Specification.

The Quality Control Department is responsible to surveil the operations of the plant to assure adherence to the operating procedures in accordance with the Operational Quality Assurance Plan and implementing procedures.

Response

Backshift and weekend visits by management and supervision are randomly con-ducted in varicus ways. Frequently, plant superviscrs work on weekends as "all as make visits to the plant on backshifts to mcDitor major plant evolu-tions (such as plant shutdowns or trip recoveries). Quality Assurance audits are also conducted en backshif ts as well as day shifts. In addition, a supervisor (licensed as a Senior Reactor Operator) is on shift along with the shift foreman and the operating crew to monitor and assess operations, including crew manning and performance, equipnent status and plant conditions.
