Identification of Occurrence:
Title:
Higher Than Allowed Quantity of NaOH in the Sodium Hydroxide Tank of the Reactor Building Spray System Type:
An abnormal occurrence as defined by the Technical Specifications, paragraph 1.8b, in that the quantity of Na0H contained in the Sodium Hydroxide Tank exceeded a limiting condition for operation as defined by the Technical Specifications, parr graph 3. 3.1. 3.b.
5 Conditions Prior te occurrence:
The reactor was at steady state power with major plant parameters as follows:

Descripticn of Incident:
Just prior to perfor=ing a routine surveillance of the Scdiu=
Hydroxide Tank, it was deternined that the surveillance procedure was in error in that it did nct acccunt for NaCH centained below the tank's zero level indicatien. The subsequent surveillance, therefore, was conducted such that it teck into acccunt MACH centained below the tank's zero level indication; and it was deter =ined that the Sodiu= Hydroxide Tank centained 17,677 pounds of NaOH, which was in violation of Technical Specification 3 3.1 3.b.
7 Designation of Apparent Cause of Occurrence:
The apparent cause of the occurrence was an incorrect surveillance trocedure which, prior to the occurrence in question, provided i= proper guidance to those responsible fer =cnitoring the Sodiu=
Hydroxide Tank para =eters.

Analysis of Occurrence:
It is believed that the higher-than-alleved quantity of NaOH in the Sodiu= Hydroxide Tank did not represent a threat to either the health or safety of the public in that the NaCH level in the Sedium Hydroxide Tank can v.ry cver certain censervative design li=its and still provide adequate pH centrol of the Reactor Spray Syste=;
and it is presently believed that the quantity of NaCH in the tank did not exceed these limits. Because such a conclusien de= ands more extensive analyses, we vill proceed with analyses that we have already begun and report to you within ten days en the results of these additional analyses.
9 Corrective Actions:
The i==ediate corrective acticn taken was to draf n a sufficient volu=e of NaCH solutien frc= the Sodiu= Hydroxide Tank so that the.
re=aining solutien would meet all applicable Technical Specification limits. Also, as a icne-ter= treventative action, tha surveillance precedure was corrected to account for the a= cunt of LuOH below the tank's zero level indication.
