Inadequate Administrative Centrol of Valve and Startup Checklists and Cperational Precedures Type: Viclation of Technical Specificaticns, paragraph 6.7.2a.5 which ecnstitutes an abncr=al occurrence as defined by the Technical Specificatiens, paragraph 1.8g, in that the inadequate Administrative Centrol of valve and startup checklists and cperaticnal procedures might have threatened to cause an unsafe condition in the operation of the plant.

(5) Ccnditiens Prior to Occurrence: Hot shutdcyn, in preparation fcr initial criticality with plant parameters as follevs:

Directorate of Licensing June 14, 19Th RC Pressure: 2155 psig RC Te=p.: 532 F FRZR Level: 100" PRZR Temp.: 6h5 F (6) Descriptien cf Occurrence: Cn June h, 197h, several cutdated valve and startup checklists from outdated procedures were fcund to have been used in the course of plant heatup, and, in ene case, an cut-dated procedure was used to operate a system. The cutdated check-lists and procedure were prc=ptly replaced with current ecpies.

A ccmparison of current and previcus editions was made and no unsafe ccnditicns were found to exist as a result of having used the outdated editiens.

(T) Apparent Cause of Occurrence: Personnel, in that although a precedure exists which requires perscnnel using procedures er parts of procedures (which includes checklists) to ensure they are current, the cperaticns persennel involved in this incident did not ensure that the precedures they were using were the current editions.

Another cause which centributed to the cccurrence was trccedure in that responsibilities were not well enough defined regarding the maintaining of procedure files.

(8) Analysis of Cecurrence: This cccurrence did not result in a threat to public health or safety in that no unsafe conditions were caused to exist.

(9) Corrective Acticns:  !=nediate corrective actions were as described in the above analysis.

The Plant Operations Review Cc==ittee met promptly after the incident and recen= ended to the Station Superintendent thr.' :

a. a cceplete audit be ccnducted of all precedure files , to include:

(1) replacing outdated precedures with current editions, (2) destroying extra copies of procedures and extra ecpies of parts cf procedures, and (3) conducting the audit en a periodic basis, and

b. a =emo be written to (1) define those individuals responsible fcr maintaining up-tc-date procedure files, and (2) reiterate to plant persennel the importance of assuring they use cnly current precedures. ,
