Identification of Occurrence:

Title:

Failure of Makeup and Purification Pu=p 1A due to inadvertent operation with an isolated suction line.

Type:

An abnormal occurrence as defined by the Technical Specifi-cations, paragraph 1.8d, in that the rump failure may have threatened to cause an Engineered Safeguard System to be incapable of performing its intended function.

Description of Occurrence: At 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br /> on June 11, 197h, Makeup and Purification Pump 1A started on a false F.ngineered Safeguard signal and ran for a short period of time with its suction valve closed. The pump was i==ediately secured and checked for damage.

It was found that the pump shaft could not be turned by hand, as had been done the previous night, indicating that there was inter-nal pump damage.

Designation of Apparent Cause of Occurrence: At the time of the occurrence, investigative maintenance was being performed on the partially energized relay lx3A/RCA, which forms part of the Engi-neered Safeguard System's manual actuation logic. Due to their close proximity, relays 1xlA/RCA and 1xlB/RCA, while in a partially energized condition, could conceivably be actuated by an uninten-tional bump. It is believed that in the present instance this is what happened.

A further investigation revealed another ccndition which apparently compounded the problem.

It was found that incandescent bulbs had been installed on the backlighted switches of the console instead of the prescribed neon bulbs and that incorrect resistors had also been installed. As a result, relays 1x3A/RCA, lxlA/RCA, and lx13/RCA probably had higher-than-ncr=al quiescent voltages, which could have further resulted in relay instability.

An unusual service condition compounded the problem even further in that maintenance was being performed on Makeup and Purification Pump 1C.

At the time, the suction piping for Makeup and Purifica-tion Pump 1A was lined up only to the 3 crated Water Storage Tank through the nor= ally closed, remote-operated MU-V-lhA Valve. This valve normally opens en an Engineered Safeguards signal from the 1x3A/RCA relay; hcvever, this valve did not actuate, even though it was partially energized.

Analysis of Occurrence:

It is believed that the failure of Makeup and Purification Pump 1A did not represent a threat to the health and safety of the public, because the plant's safeguards syste=

vas not adversely affected, in that during an LOCA only one of the injection lines is required to provide core protection.

Corrective Actions:

I==ediate corrective actions, in additien to those described above, involved a.

removing the incandescent bults and replacing them with the proper necn bulbs; and b.

initiating a test to verify that the manual actuation relays were performing their desi gn functions.

The Plant Operations Review Committee (PORC) met prc=ptly after the incident and reco== ended the follcving long-tern actions, with which the Station Superintendent concurred:

a.

replace all resistors, as necessary, with ones having the proper resistance; b.

initiate an investigation on the advisability of adding addi-tional pump protecticu, such as a low-suctica pressure trip.

As of this date, action has been initiated on both of these items by the Station Superintendent.
