This situaticn related to Ccndensate Storage Tank 13 Diaphrag.
Descrintien of Incident During ecmplete draining of Condensate Storage Tank 13 to the condenser hotwell, the diaphrag: vas sucked over the sucticn screen cutting off flow. The condenser vacuun acting cn the diaphragm collapsed the suction screen which resulted in the diaphrag. being punctured in several places.
Corrective Action Taken The tank was opened to inspect the suction screen which was found to be collapsed and a new screen was designed by the Architect Engineer which vill prevent the diaphrage from blocking the suction and also prevent the diaphragm Orca being punctured.
The disphragm was repaired and the tank put back in service.
The new designed suction screen vill also be installed in Condensate Storage Tank 1A to preclude this type of incident from occurring. As a further precaution, en operating limit has been imposed by ::etropolitan Edison Company to preclude the diaphragn frc= being in contact with the suction screen.
