This refers to your application dated May 9,1979 requesting approval of your Quality Assurance (QA) program as meeting the QA program requirements of 10 CFR 971.51.
In connection with our review, and as discussed by telephone on May 18, 1979 between our Mr. Spraul and your Mr. Carpentier, we need the information identified in the enclosure to this letter. Please submit seven copies of your response to the enclosed request for additional infomation within 30 days from the date of this letter. Also, please delete the " Radiation Survey Report of Transportation Package" from the application. While there must be supportintj written procedures to implement your QA program, these procedures and forms should not be submitted as part of your QA program plan.
We recognize the infrequency of your use of the QA program, but we do require an acceptable program plan.

Please provide an organization chart which shows, by title, the reporting relationship of the Radiographic Administrator, Radiographers, and Personnel Department.
2.
The area to be audited is too limited.
Provide a statement that each activity will be audited at least once each year. The activities audited should include the related activities of the Radiographic Administrator and ",adiographers, instrument calibration, certification of personnel, e tc. Annual audits are not required for years that radioactive material is neither received nor shipped.
