1.0 Scope 1.1 This Quality Assurance program is established to ensure adequate and proper control over the shipment and receiving of Radioactive materials in their shipping packages.
1.2 It is not our objective to manufacture, repair, modify, etc. any shipping packages. We will only use shipping packages (which have been approved by the N.R.C.) as supplied by the manufacturer of the source or source exposer division.
NW1 DOCUMENT OA NRC7068-PTRM For Packaging and Transportation of Radioactive Material, O Revision, June 24,1978 2.0 Quality Assurance Criteria for Receiving Radioactive Material Packago 2.1 When packaged radioactive materials are received by NWI, the following procedure is to be observed to ensure a safe and adequate plan of receival.
2.2 Procedure for Receiving Packages of Licensed Material 2.2.1 Arrange to receive notification from the carrier, of the arrival of the package, at the time of arrival.
2.2.2 Radioactive material shall be picked up from the carrier terminal expeditiously upon receipt of notification, from the carrier on its arrival.
2.2.3 A visual inspection of the shipping package shall be made and recorded on Form NWl-1.
(See Exhibit 1) 2.2.4 As soon as practical, but no longer than three hours upon receipt of radioactive material, the external surfaces of the package shall be monitored, and recorded on Form NWl-1 along with the date, source model and serial numbers, time of notification, time of pick-up, etc., shall be recorded.
2.2.5 Any radioactive material found to be in excess of that required in Part 20.205, shall be immediately reported to the appropriate NRC regional office.
NW1 DOCUMENT OA NRC7068-PTRM For Packaging and Transportation of Radioactive Material, O Revision, June 24,1978 3.0 Quality Assurance Criteria for Shipping Packages of Radioactive Material 3.1 When packages of radioactive material are shipped by NW1 the following precedure is observed to ensure a safe and adequate plan of shipment.
3.2 Procedure for Shipping Packages of Radioactive Material 3.2.1 Notify source manufacturer to arrange receival of depleted or malfunctioning source.
3.2.1.1 Prior to shipping a visual inspection of the source container and shipping package shall be made. (See Form NWl.2, Exhibit 2) 3.2.1.2 No shipping will be permitted in a shipping package that is damaged, unsealed or unproperly marked with the required DOT labels.
3.2.2 Proper shipping certification forms that are supplied by the shipper, must be correctly completed. (See Form NWl-3, Exhibit 3)

4.0 Receival of Damaged Shipping Package 4.1 Upon receival of a damaged shipping package, the following procedures shall be imposed:
4.1.1 All unauthorized personnel shall be removed from the area; 4.1.2 Shipping package shall be monitored with a calibrated survey meter to ensure that the contents are still in a safe position within the source device; 4.1.3 The standard procedure for receiving packages of licensed material shall be followed. (See Section 2) 4.1.4 The source exposure devise shall be removed from the shipping package and monitored with a calibrated survey meter. At this time, the exposure device is visually inspected for damage.
4.1.5 Source manufacturer shall be notified as to the extent of damage and the required corrective action to be taken.
NW1 DOCUMENT QA NRC7068-PTRM For Packaging and Transportation of Radioactive Material, O Revision, June 24,1978 5.0 Document Control 5.1 All documentation on shipping and receiving of radioactive materials shall be controlled by the Radiation Safety Officer in a designated area for authorized personnel only.

6.2 Re-audits shall be performed whenever a deficiency is found.
