t ENCLOSURE 1 University of Utah Docket No. 50-72 NOTICE OF VIOLATION Based on the results of the NRC inspection conducted January 23-26, 1979, it appears that certain of your activities were not conducted in full compliance with your reactor license or NRC requirements as indicated below:
1.
Technical Specification 6.2, Procedures, states in part that
" Detailed written procedures shall be provided and followed for the following reactor operations...."
Contrary to the above requirement, procedures contained in the
" Nuclear Engineering Laboratory Operations Manual" were not followed in that:
a.
Radiation surveys around reactor seals were not conducted as required by Section 12.2(5) of the Operations Manual.
b.
Time entries were not made for various evolutions recorded in the operating log as required by Section 12.2(7) of the Operations Manual.
c.
The monthly reactor inspection required by Section 12.8.2 of the Operations Manual was not performed in February 1978.
This is an infraction.
2.
Technical Specification 6.3.l(3) requires that records of tests and measurements performed pursuant to the Technical Specifications be prepared and retained at the facility for five years.
Contrary to the above, substantiating data for reactivity measure-ments made pursuant to TS 5.2, and reported in the 1977 annual operating report, could not be provided by licensee representatives.
This is a deficiency.
