Appendix A NOTICE OF VIOLATION Cincinnati Gas and Electric Docket No. 50-358 Company Based on the results of an NRC inspection conducted on January 22-26, 1979, it appears that certain of your activities were in noncompliance with NRC requirements as noted below.
Items 1 and 2 are infractions; items 3 and 4 are deficiencies.
1.
10 CFR 50, Appendix B, Criterion VI states, in part:
" Instructions, procedures... including changes, are reviewed and approved f or release by authorized personnel and are distributed to and used at the location where the prescribed activity is performed."
Wm. Zitrer Nuclear Power Station, Unit 1, FSAR, Chapter 17, Paragraph 17.2.11 states, in part: "Each test is accomplished and documented in accordance with written controlled test procedures."
Contrary to the above, unapproved Instrument Calibration Procedure No. IC-GGP-P101 was used to Calibrate Instrument No. 1Pi-CM010.

10 CFR 50, Appensix B, Criterion V, states, in part, " Activities affecting quality shall be prescribed by documented instuctions, procedures or drawings.
. and shall be accomplished in accor-dance with these instructions, procedures.
Kaiser Procedure No. SPPM 3.3R4, Paragraph 6.9 states, " Damaged and discarded welding materials, sub ends must be clear from work area."
Co - 3ry to the above:
a.
At the 475' level in the Reactor Building, two dozen stubs and damaged E7018 electrodes were found on the floor. Eleven unused electrodes E7018 were found in cable tray No. 2118K about 10' above floor level.
b.
At the 50' level, both E6010 and E7018 unused weld rod was lying on scaffolding with no welding in the area in progres,.

Contrary to 10 CFR 50, Appendix B, Criterion XI, states, in part,
" Test results shall be documented and evaluated to assure that test requirements have been satisfied."

Wu ~. Zimmer Nuclear Power Station, Unit 1 FSAR, Chapter 17, Pa.agraph 17.2.11, states, in part, " Test results are documented, analyzed, and evaluated by the individual or group that prepared the test procedure to insure that these requirements are satisfied."
Contrary to the above, test data sheet for instrument IE22-N004 calibrated in January, 1979, had been reviewed and accepted by the I6C supervisor even though calibration for test instruments TDV-054 and TGA-193 had allegedly expired in October, 1978, and December, 1978, respectively.

10 CFR 50, Appendix B, Criterion XII, states, in part, " Measures shall be established to assure that... gauges, instuments.
used in activities affecting quality are properly controlled, calibrated and adjusted at specified periods to maintain accuracy within necessary limits."
The Wm. H. Zimmer Nuclear Power Station, Unit 1, FSAR, Chapter 17, Paragraph 17.1.12.2, states that, " Measuring and test equipment is calibrated and adjusted at specified periods to maintain accuracy within necessary limits,...."
Contrary to the above:
a.
Calibration stickers affixed to three (3) weld rod hold ovens indicated that they had been calibrated on September 20, 1978, even though the licensee stated that they did not have a direct method of measuring oven temperature, b.
Measures had not been established to assure that instruments used in activities affecting quality were properly controlled and calibrated at specified periods in that two (2) of the wel'd rod hold ovens were past due the specified calibration dates.
The inspection showed that action had been taken to correct Item 4b noncompliance and to prevent recurrence. Consequently, no reply to Item 4b is required and we have no further questions regarding this matter at this time.
