99900029/78-02 fl0TICE OF DEVIATION Based on the results of an flRC inspection conducted on flovember 2-3, 1978, it appeared that certain of your activities were not conducted in full compliance with flRC requirements as indicated below:
The Youngstown Welding & Engineering Company's letter dated June 16, 1978, in response to flotice of Deviation, Items A.1, A.2, and B., enclosed with the flRC inspection report cover letter dated April 13, 1978, indicated that corrective and preventive actions would be taken during April and May 1978, for these items.
Contrary to the above, the following conditions were found during the current inspection:
1.
Item A.1: The corrective action has not been taken in that five areas of the QA Manual which were previously not audited are still not audited.
2.
Item A.2:
The preventive action to assure that the AtlI reviews the monthly audit reports by inclusion of Afil review in the audit checklists has not been done.
3.
Item B:
The preventive action to assure that the training and indoctrination outline is followed in conducting training sessions required by paragraph 2.7 of the QA Manual has not been done in that no training has been performed under the outline dated August 11, 1978.
