Facility: Trojan Nuclear Plant, PO Box 439, Rainier, Oregon 97048 4
Identification of Occurrence An inadvertent actuation of the ECCS safety injection occurred while troubleshooting a turbine generator control circuit.

Conditions Prior to Occurrence The plant was critical, starting up (Mode 2) at 1.55 ci full power.
All primary system parameters were at their normal operating values.

Description of Occurrence During the recovery from a plant trip while troubleshooting the EHC system electronics, a reactor trip and safety injection occurred. The backup acceleration amplifier card was pulled out of the rack causing the main steam control valves to go wide open which resulted in a high steam flow / low steam pressure safety injection.

Designation of Apparent Cause of Occurrence The apparent cause of the occurrence is a faulty procedure. The I&C Technician was following the General Electric Company's troubleshooting procedure step by step when the event occurred.
8.
Analysis of Occurrence This occurrence had no effect on either plant or public safety. All systems and components that are called upon to act during a safety injection functioned as they were intended.
9.
Corrective Action General Electric Company has changed the procedure to prevent this from recurring.
