Colt Industries Trent Tube Division of Crucible, Inc.
Docket No. 99900314/79-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on August 27-31, 1979, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or dra* wings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Deviations from these requirements are as follows:
A.
The QA Manual paragraph 18.2 states in part, " Inspectors, Technicians and Supervisors shall initiate Rejection Tags.
. for nonconformities.
. A copy of the tag shall be inserted in the envelope containing the Mill Order and remain with the material until the rework is complete and acceptance verified at final inspection."
Contrary to the above, four (4) stainless steel pipes, 4.500" 0.D. X.237" wall X 21' long, were observed in a reject rack without an accompanying Rejection Tag and Mill Order.
Corrective action was implemente3 prior to the end of the inspection by attaching a copy of the Rejection (rework)
Tag No. 4969 to these pipes to control required rework.

B.
The QA Manual paragraph 17.1 states in part, "... The.
. instruments and other devices listed in the calibration log shall be calibrated
." The Calibration Log for 1979 lists " Welding Volt Meters" as devices to be calibrated on a 6 month frequency.
Contrary to the above, the voltmeters in the two (2) automatic voltage control regulators used on the automatic girth welding machine No. 718, were not calibrated.

The ASME Code NB/NC-2180 states in part, "When heat treating temperature or time is required by the material specification and the rules of this Sub-section, the heat treating shall be performed in temperature-surveyed and calibrated furnaces.
Contrary to the above, it could not be verified that the heat treatment of SA-358 Class 1, ASME Code Section III Class 1 and 2, T-304 piping for Mill Orders No. IC5-45150-9 and IC5-41199-8, was performed in a temperature surveyed and calibrated furnace.
No records of temperature surveys and calibration data for the furnace instruments were made available.
