Appendix A NOTICE OF VIOLATION CTL Engineering, Inc.
License No. 34-18533-01 Based on the inspection conducted on October 24, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Item 1 is an infraction and item 2 is a defeciency.
1.
License Condition No. 13.A requires that sealed sources containing licensed material shall be tested for leakage and/or containment at intervals not to exceed six months.
Contrary to the above, the Troxler gauge, Serial No. 5330, containing a 40 millicurie americium 241 sealed source and an 8.3 millicurie cesium 137 sealed source, was not leak tested between June 16, 1978 and February 9,1979, a period in excess of six months.

10 CFR 20.401(b) requires that each licensee shall maintain records showing the results of monitoring required by 10 CFR 20.205(c).
Contrary to the above, no record was maintained showing the results of monitoring which was performed at the time of receipt of a Troxler gauge, Serial No. 5430, which contained a 40 millicurie americium 241 sealed source and an 8.7 millicurie cesium 137 sealed source.
