Gibbs and Hill Incorporated Docket No. 99900524/79-02 NOTICE OF DEVIATION Based on the results of a NRC inspection conducted on July 9-13, 1979, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
10 CFR 50, Appendix B, Cr iterion V states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings...and shall be accomplished in accordance with these instructions, procedures or drawings."
Comanche Peak Steam Electric Station (CPSES) Quality Assurance Procedure QA-7 (Issuance, Modification and Control of Project Procedures Manual), Revision 2, dated April 1979, states in part,
" Revisions to a procedure will be made in the following manner:
...Where only limited text changes are made, they will be clearly indicated by a solid line adjacent to the change on the right hand margin of the page and include the revision number."
Contrary to the above, Design Control Precedure DC-2 (Design Description Procedure), Revision 4, dated May 1979, was revised with limited text changes and the solid line on the right did not include the revision number.
