Dear Sir:

p Docket No. 50-289 Operating License DPR-50 This letter is submitted to you pursuant to the Three Mile Island Unit 1 Technical Specifications (TMI 1 T.S.) section 6.7.2.a.2, and addresses unplanned releases of radioactive material that occurred on October 6, 1974 (Nonroutine Report Number 74-02); October 8,1974 (Nonroutine Report Number 74-03) ; and October 9-10, 1974 (Nonroutine Report Number 74-04).

Furthermore, the information provided herein is also applicable to other incidents which occurred on September 11,14, and 15,1974, none of which were determined to be sufficiently above background to qualify as unplanned releases of radioactivity, and the incidents of September 5, 6, and 7,1974, which were previously reported in the Nonroutine Report No. 74-01 ten day letter dated October 3,1974. Also, this information is being submitted to you one day af ter the Nonroutine Report Number 74-02 ten day due date of October 16, 1974, in accordance with the Octvber 16, 1974, telephone conversation between D. N. Grace of our Licensing Section and D. Jaffe of your office.

All cf the above referenced incidents were caused due to problems associated with the Reactor Coolant Waste Evaporator and associated systems, and in all the incidents immediate corrective actions were successfully executed to protect plant personnel and to contain radioactive materials. Furthermore, it has been determined for all three incidents that:

a.

there were no threats to the health and safety of the public in that 1.

none of the TMI 1 T.S. limits were exceeded, and 2.

no personnel were over-exposed, and b.

interim corrective and preventative actions--which include upgrading of monitoring and personnel radiation protection requirements--are being taken, and will remain in effect until all permanent corrective and preventative actions 1(Jgl(}

can be fully implemented.

4,e We trust that this letter, together with information previously reported to representatives of the Directorate of Regulatory Operations Region 1 Office, are sufficient to answer any immediate concerns that you may have; and we presently anticipate being able to provide you with a final report which addresses all the reporting requirements of the TMI 1 T.S. and any reports that may be required pursuant to 10 CFR 20 by October 24, 1974.
