Oregon Graduate Center 19600 N. W. Walker Road Beaverton, Oregon 97005 License No. 36-18039-01 Notice of Violation Based upon the results of the NRC inspection conducted on June 5,1979, certain activities under your license appear to be in noncompliance with NRC require-ments as identified below:
A.
Contrary to License Condition 12, at the American Samoa site, the two nickel 63 foils, containing 15 millicuries each and identified as C0 708 and C0 709, were used by persons other than as specified in the license.
This is an infraction.
B.
Contrary to 10 CFR 20.407, a personnel monitoring report for the 1978 calendar year was not submitted to NRC.
This is a deficiency.
