Colt Industries Fairbanks Morse Engine Division Docket No. 99900300/78-01 fl0TICE OF DEVIATION Based on the results of an flRC inspection conducted on October 2-6, 1978, it appeared that certain of your activities were not conducted in full accordance with URC requirements as indicated below:
A.
Fairban;s Morse Engine Division's corrective action response letter of November 21, 1977, states in part, " Additional Audits (in excess of the requirem:nts of Section 18 of the Quality. Assurance Manual) are being conducted to monitor performance to the requirements of paragraphs 5.4.1.3 and 10.4.4.2 of the Quality Assurance Manual."
Contrary to the above, additional audits (in excess of the require-ments of the Quality Assurance Manual) were not conducted to monitor performance to the requirements of paragraphs 5.4.1.3 and 10.4.4.2 of the Quality Assurance Manual.
B.
Fairbanks Morse Engine Division's corrective action response letter of November 21, 1977, states in part, "The dispatcher will verify that the operator has both the white copy of the operation sheet and the Engineering drawing. This system will be fully.~mplemented in December 1977."
Contrary to the above, the operator did not have the app. 1 " ble drawings for Operation sheet, dated August 24, 1978, for Poi. Number 12609671.
The Operation Sheet required eight (8) drawings; of these, three (3) were the correct revision while four (4) were not, with one missing from the manufacturing order packet.
Examples: Drawing No. P08997, Revision 3 was required but Revision 2 was in 1e package; Orawing No. P12607417, Revision 2 was required but Revision 1 was in the package; Drawing No. P08774, Ravision 3 was required and was in the package; Drawing No. 11871925, Revision 3 was required but was not in the package.
C.
Criterion VI of Appendix B to 10 CFR 50 states in psrt, " Measures shall be established to control the issuance of documents, such as instructions, procedures, and drawings, including changes thereto, which prescribe all activities affecting quality. These measures shall assure tha'. documents, including changes, are reviewed for adequacy and approved for release by authorized personnel...."
Paragraph 6.4.2.1 of the Quality Assurance Manual, Revision 11, dated April 1978, states in part, " Changes to Engineering drawings and data requires the processing of a change order request...

. Contrary to the above, Drawing No. 11871441 (Test Procedure), Revision 3 reflects a change of paragraph 1.A.2.(b)(8) that does not appear on the identified Change Order No. P6842.
D.
Criterion III of Appendix B to 10 CFR 50 states in part, "The design control measures shall provide for verifying or checking the adequacy of design, such as by the performance of design reviews, by the use of alternate or simplified calculational methods, or by the performance of a suitable testing program.
Design changes, including field changes, shall be subject to design control measures commensurate with those applied to the original design..."
Paragraph 2.3 of Exhibit IIA-2 to Specification No. 2447.300-241, dated September 20, 1973, states in part, "The Contractor / Vendor shall have written procedures which will provide for verifying or checking the adequacy of design and subsequent changes or revisions thereto, such as by performance of design reviews, alternate or simplified testing program."
Contrary to the above, verification of design and subsequent changes had not been performed.
