O Appendix A NOTICE OF VIOLATION Chicago State University License No. 12-17318-01 Based on the inspection conducted on January 22, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Items 1, 2, 3, and 7 are deficiencies, items 4 through 6 are infractions.

10 CFR 19.ll(a) and (b) requires that current copies of Part 19, Part 20, license conditions, documents incorporated into the license, license amendments and operating procedures be posted or that a notice describing these documents and where they may be examined, be posted.
Contrary to these requirements, on the date of the inspection, January 22, 1979, neither the documents nor notice were posted.

10 CFR 19.ll(c) requires that Form NRC-3, " Notice to Employees,"
be posted by the licensee wherever individuals work in or frequent any portion of a restricted area.
Contrary to the above, on the date of inspection, January 22, 1979, there was ne posting of Form NRC-3 in laboratory areas frequented by licensee representatives.

10 CFR 20.203(e) requires that each area or room in which licensed material is used or stored and which contains any radioactive material (other than natural uranium or thorium) in an amount exceeding ten times the quantity of such material specified in Appendix C of this part shall be conspicuously ported with a sign or signs bearing the radiation caution symbol and the words:
" Caution, Radioactive Material (s)".
Contrary to the above, the Isotopes Lab containing radioactive material in an amount exceeding ten times the quantity specified in Part 20, Appendix C, was not posted with the appropriate symbol or sign. In addition, both the Isotopes Lab and Countroom (containing no byproduct material) were inappropriately posted as " Radiation Areas."

Condition 13 of your license requires that licensed material be possessed and used in accordance with statements, represen-tations, and procedures contained in application dated February 24, 1977.
Contrary to Supplement D, Item 11 of these procedures which requires a six month calibration frequency for your Eberline E-120 survey meter and an annual calibration frequency for your Victoreen R-Meter, it was determined through statements of former licensee representatives and the absence of records, that no instrument calibration program was ever maintained.
A sticker on the E-120 survey meter indicated that it had not been calibrated since May 27, 1977.
In addition, on the date of the inspection, the survey meter was inoperable in that the battery was dead.

Condition 13 of your license requires that licensed material be possessed and used in accordance with statements, representa-tions and procedures contained in application dated February 24, 1977.
Contrary to Supplement F, Item 14 of these procedures which requires that the radioisotopes lab, counting lab, contiguous hallways and laboratories be surveyed weekly, it was determined through statements of forner licensee representatives and the absence of records, that no survey of these areas has ever been performed.

Contrary to Supplement D. Item 12 of these prc.adures which is a commitment for film badging all radiation workers, one individual involved with licensed byproduct materials was found to have never been issued a badge.
In addition, this individual stated that he had used the badge of a terminated employee whose film badge service was not discontinued.
