Docket Number 99900051 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on September 13-14, 1978, it appeared that certain of your activities were not conducted in full com-pliance with NRC requirements as indicated below:
A.
Criterion V of Appendix B to 10 CFR 50 and Paragraph NCA-4134.5 of Section III to the ASME Code states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings... and shall be accomplished in accordance with those instructions, procedures, or drawings...."
VEC QA Manual Section VEL-QC-506A, paragraph 3.1 states, " Purchase requisitions are prepared and issued by the designated Planner on the form shown on this procedure (Exhibit 31)..."
Contrary to thesis requirements, purchase requisitions were not prepared
'for P0 C-5277 dated April 19, 1977, for 46 SA-105 and SA-182 (ASME Section II 1974 Edition plus Winter '75 addendum) 10" through 16" valve forgings and P.O.C-5276 dated February N, 1978, for 156 SA-182 (ASME Section II 1974 Edition plus current auuenda) 6" th ough 14" valve forgings.

Criterion V of Appendix 3 to 10 CFR 50 and Paragraph NCA-4134.5 of Section III to the ASME Code states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings... and shall be accomplished in accordance with these instructions, procedures, or drawings...."
VEC QA Manual Section VEL-QC-750A, paragraph 10e states, "The Quality Audit Manager schedules monthly Management Audit Meetings to discuss the findings made on the Audit reports. Minutes of the meeting stating corrective actions and commitment dates are prepared and distributed by the QA Department to all attendees and to the President and General Manager. "
Ccatrary to the above requirements, minutes of the Monthly Management Meeting did not address the corrective action and establish commitment dates for the finding (Item 12.b) in the audit reoort enticled " Audit Schedule No.18" dated August 15, 1978, which stated " Audits were lot performed annually."
C.
Criterion V of Appendix B to 10 CFR 50 and Paragraph NCA 4134.5 of the ASME Code state in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of 79020903 7 3 a type appropriate to th2 circumstances and shall te accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quanti-tative or qualitative acceptance criteria for detemining that important activities have been satisfactorily accomplished."
VEC's QC Procedure VEL-QCl-138A, Subparagraph 5, " inspection," states that the Patrol inspector shall stamp the welding logbook after verification that QC Requiremants tre acceptable.
VEC's welding procedure specification (WPS) No. VEL-P-642 (P8)
Large Parts Revision O fer vielding process PAW requires an interpass g
temperature of 250-350 F to be maintained during the welding process.
Contrary to the above, it was observed that the Patrol Inspector is not stamping the welding logbook, and the interpass temperatures for Procedure VEL-P-642 are not being monitored.
