TEREX 10 CFR71 QUALITY ASSURANCE PROGRAM FOR INDUSTRIAu RADIOGRAPHY uICENSE A.
Organization 1.
Terex div. retains the responsibility for the quality assurance program. However, the design and fabrication of radioactive material shipping packages shall not be conducted under this program.
2.
The Quality Assurance Program is implemented using the following organization:

- 3. The radiation safety officer is responsible for the overall r-administration of the part 71 Quality Assurance requirements, including training, certification, document control and auditing.
- 4. The Asst. Radiation Safety Officer is responsible for these same areas when the radiation safety officer is absent from the plant.

The radiographers are responsible for handling, storing, shipping, inspection, test, operating status and record keeping.
B.
Quality Assurance Program
- 1. The Management of Terex Div. GMC will extablish and implement the Quality Assurance Program. Training and experience will be required for all Quality Assurance functions according to writtem procedures. Quality Assurance Program revisions will be made according to written procedures. The Quality Assurance Program will ensure that all defined Qc procedures, engineering procedures and specific provisions of the package design approval are satisfied.
The Quality Assurance Program will emphasize the control of the characteristics of the package which are critical to safety.
- 2. The Radiation Safety Officer shall assure that all radioactive material shipping packages are designed and manufactured unde a Quality Assurance Program approved by the Nuclear Regulatory Commission for all packages designed or fabricated after 1 July 1978. This requirement can be satisfied by recieving a certification to this effect from the manufacturer.
C.
Document Control
- 1. All documents related to a specific shipping package will be controlled thru the use of written procedures. All document changes will be performed according to written procedures.
- 2. The Radiation Safety Officer will assure that all Quality Assurance functions are conducted in accordance with the latest applicable changes to these documents.
D.
Handling, Storage and Shipping
- 1. Safety restrictions shall be followed concerning the handling, storage and shipping of packages for certain special form radioactive material. Shipments will not be made unless all tests, certifications, acceptances, and finalinspections have been completed. Work instructions will be provided for handling storage and shipping operations.
E.
Inspection, Test and Operating Status
- 1. Inspection, test and operating status of packages for certain special form radioactive material will be indicated and controlled by writ-ten procedures. Status will be indicated by tab, label, marking or log entry. Status of Non-Conforming parts or packages will be positively maintainged by written procedures.
- 2. Radiographers shall perform the regualtory required inspections and tests in accordance with written procedures. The Radiation Safety Officer shall ensure that these functions are performed.
4 Quality Assurance Records
- 1. Records of Package approvals (including references and drawings),
inspections, tests, operating logs, audit results, personnel training and qualifications, and records of shipments will be maintained.
Descriptions of equipment and written procedures will also be main-tained.
- 2. These records will be identifiable, retrievable and maintained in accordance with written procedures.
- 3. A list of these records, with their storage location, will be kept by the Radiation Safety Officer.
G.
Audits
- 1. Scheduled audits of the Quality Assurance Program will be performed using written checklists. Results of these audits will be reported to management.
- 2. Audit results will be evaluated and deficiencies corrected.
- 3. The extent and frequency of the audit will be dependent on the safety significance of the activity being audited but the frequency shall be no less than once per year.
- 4. The audit reports will be maintained as part of the Quality Assurance records
- 5. Members of the audit team shall have no responsibility in the activity being audited.
