Dear Mr. Sutherland:
In response to your letter dated November 8, 1978, in which you refer to the inspection conducted by Mr. R. L. Woodruff of your Office we submit the Plan of Correction on the infractions and deficiencies of license number 5208827 and license number SNM-1407 for the Auxilio Mutuo Hospital.
Item A of License Number 52-08827-02: This item refers to Molybdenum-99 breakthrough should be performed on the cluate from each elution of the generator, the test has been ordered to Picker del Caribe, Inc.
- However, the company submitted information as to the none availability of such kit in Puerto Rico and has prom #. sed its delivery no later than Decem-ber 30, 1978. We are proposed to start tests as soon as materials ordered arrive.
Item B of License Number 52-08827-02: This item refers to subpart 1, 2, and 3, which have been posted as specified in your comments.
Your license SNM-1407, Item C: We have started and already have ac-counted three patients which have nuclear pacemakers.
(Julia Guzman, Delia Serrano, Josefina MiEndez) Follow up is being made to procur correct addressess and telephone numbers.
We intend to monitor patients nore closely in order to continue with Follow up and replamment procedures for the nuclear pacemaker during the life of the patients. We are in the process of establishing procedures (Continued.....)
