This outlined Quality Assurance Program describes those quality control systems employed by Capital X-Ray 3ervice for the procurement, receipt, radiation levels surveying, storage, operation, transporta-

  • ion for use, preparation for shipment, and the delivery to carriers for shipment of those Type "B" Radioactive Material Shipping Packages for which Capital X-Ray Service maintains Certificates of Compliance in accordance with U.S.N.R.C. Byproduct Material License #35-11114-01:

The design, fabrication, quality certification, repair, radiation levels surveying, preparation for ship"ent and the delivery to carriers for shipment of radioactive material shipping packages to Capital X-Ray Service shall be conducted only by the Manufacturer in accordance with an approved Quality Assurance Manual and shall under no circumstances be conducted by Capital X-Ray Service.

The actual shipment of radioactive material shipping packages to Capital X-Ray Service from the Manufacturer and from Capital X-Ray Service to the Manufacturer shall be conducted only by commercial freight carriers who are regulated by agencies of the U. S. Department of Transportation and/or the United States Nuclear Regulatory Commission.

The entire Management of Capital X-Ray Service has participated in the preparation of this Quality Assurance Manual.

M OUALI*Y ASSURANCE PROGRAM 2.1 The Management Staff of Capital X-Ray Service establishes and implements the systems within this Quality Assurance Manual and those systems within USNRC License No. 35-11114-01.

2.2 If specific training for any cuality assurance function is required, that training shall be performed prior to the engage-ment of these functions by personnel.

2.3 Revisions to the Quality Assurance Program shall be made only according to established procedures.

2.4 The Director of Quality Assurance shall obtain from the manu-f acturers of radioactive material shipping packages, written evidence that those packages are designed, fabricated, tested, repaired, and prepared for shipment to Capital X-Ray Service in accordance with approved quality control procedures.

The manu-facturers must also submit the actual shipping classifications for each approved pa'ckage design.

2.5 The Quality Assurance Program shall also emphasize control of those characteristics of any package which are critical to safety.

3.0 DOCUMENT CON'"ROL 3.1 The maintenance of, revision of, and the control of all"docu ments related to USNRC Byproduct Material License #35-11114-01 and all documents related to specific radioactive material shipping packages is a functional responsibility of the Director of Quality Assurance / Radiation Safety Officer.

3.2 The Director of Quality Assurance / Radiation Safety Officer shall assure that control copies of up-to-date procedures relating to specific radioactive material shipping packages are submitted only to:

4.0 PROCUREMENT, RECEIPT, RADIATION LEVELS SURVEYING, STORAGE, OPERATION, TRANSPORTATION FOR USE, PREPARATION FOR SHIPMEh",

INSPECTION, AND DELIVERY TO CARRIERS FOR SHIPMENT 4.1 The step-by-step procedures for the procurement, receipt, radia-tion levels surveying, storage, operation, transportation for use, preparation for shipment, inspection, and delivery to carriers for shipment of radioactive material shipping packages for which Capital X-Ray Service maintains Certificates of Com-pliance are contained within the " Internal Inspection System or Other b'anagement Control" and " Operating and Emergency Proce-dures" portions of Capital X-Ray Service's USNRC Byproduct Material License No. 35-11114-01.

These step-by-step procedures shall be followed without any deviation.

4.2 Shipments of radioactive material shipping packages, whether or not they contain a sealed source of byproduct material, shall

M QUALITY ASSURANCE RECORDS 5.1 The Director of Quality Assurance / Radiation Safety Officer, George W. Johnson, shall maintain for reference and inspection all records and reports which includes package approvals pur-suant to this Quality Assurance Manual and USNRC Byproduct Material License No. 35-11114-01.

6.0 AUDITS 6.1 Audits of the Quality Assurance Program shall be performed by Thomas W.

Payne, General Manager, on established schedules.

An audit check list, copy attached, shall be documented and the audit results shall be evaluated and nonconforming areas shall be corrected im.ediately.
