Dear Mr. Stello:

Tnis letter is in response to the Notice of Violation dated Deccaber 31, 1979 which states that I was not in full compliance with the conditions of my Senior Operator License (No. SOP-2768-1) in that I approved an equiptent clearance permit for the refueling water storage tank isolation valve [MOV-CH-115D].

As the Senior Licensed Operator directing the licensed activities of licensed operators on November 27, 1979, I would like to provide you with soce insight as to the circumstances which I believe caused this item of noncompliance and to inform you of the actions I have taken to prevent recurrence of a similar noncompliance.

During the morning of Novecber 27, 1979, as' Nuclear Shift Supervisor, I properly relieved the previous (0000 to 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br />) Nuclear Shift Supervisor. He provided me with all the necessary information to start the day's operation and maintenance activities of the station originating from and through the Unit 1 control room. At this time, I was also informed that the No. 1 Emergency Diesel Generator was out of service for an eighteen month preventive maintenance procedure. During this time of day, I was directly involved with station activities not related to_ routine operation and maintenance of the station. These activities included security functions and providing Duquesne Light personnel and construction personnel O L.

Mr. Victor Stello January 10, 1980 BVPS:DAC:1 Page 2 with keys to enter all vital areas for routine jobs and modifications that could be performed during power operation in preparation for the upcoming major outage scheduled to start December 1, 1979. My activities relating to security were greatly increased due to the influx of construction personnel who had to be authorized.(by the Nuclear Shift Supervisor) onsite to prepare for the modification outage.

Simultaneous with the above, while directing operations activities, I was in con =unication with the Electrical Maintenance Department due to th'e fact that the refueling water storage tank isolation valve [MOV-CH-ll5D]

did not perform properly Juring an Operations Surveillance Test conducted the previous day. I was concerned that we should correct this problem in a timely manner and was expecting the clearance form to arrive in the control room. As of 0825 hours0.00955 days <br />0.229 hours <br />0.00136 weeks <br />3.139125e-4 months <br />, the clearance had not arrived in the control room and I departed for the morning Planning and Scheduling Meeting. On my way out of the control room, I encountered an electrician with the clearance in hand. I approved the clearance and instructed an operator to post it on [MOV-CH-115D].

I then proceeded to the regular corning meeting. The November 27 meeting was Icnger than usual due to discussions concerning the upcoming outage and did not adjourn until approximately 1015 hours0.0117 days <br />0.282 hours <br />0.00168 weeks <br />3.862075e-4 months <br />. Upon my return to the control room, an operator raised a question about the No. 1 Diesel Generator being on clearance and hcw it af fects the operability of [MOV-CH-115B], the redundant flow path of [MOV-CE-1153], shich supplies emergency core cooling from the refueling water storage tank. It became very clear that I was in error and I icnediately centacted the electrician who signed on the clearance, requesting him to remove his operating tag. I then instructed an operator to reenergize

[MOV-CH-115D] and open the valve and then permitted the electrician to continue working on the valve operator in the deenergized open position. This action returned the high head safety inj ection Train "B" to service. I notified station management of the event and prepared a draft report of the event.

On the afternoen of November 27, 1979, being very concerned with the activitie s in the centrol room which contributed to the occurrence of this event, I requested that station staff aid operations in reducing specified activities not related to direct operation of the facility. I suggested that the duties of authorizing construction and .endor personnel, etc. onsite during the morning hours and the issuance of vital areas keys be performed by other staff members. Both of the above items were acted on by station staff and are no longer duties of the shif t supervisors.

Mr. Victor Stello January 10, 1980 BVPS:DAC:1 Page 3 During the days following the November 27 event, I participated in the Onsite Safety Ce==1ttee meetings relating to the above-described incident.

I had direct input to the administrative procedures that have been subsequently adopted and icplemented to ensure redundant, independent verification of the operability of the recaining engineered safety features whenever any safety system, or subpart thereof, is intentionally removed from service.

I have also upgraded myself and my activities in directing the licensed activities of licensed operators with on-shif t training for myself and the operators on my shif t ccacerning the use of the new administrative procedures to prevent recurrence of sitilar items of noncompliance.
