Public Service Company of Colorado Fort St. Vrain Docket No. 50-267 NOTICE OF VIDIATION Based on the results of an NRC inspection conducted September 29 - October 26, 1979, it appears that certain of your activities were not conducted in full compliance with hPC requirements as indicated below:
Technical Specification 7.4.a, Procedures, Administrative Controls, requires in part that " written procedures shall be established, implemented, and maintained.
A.
Overall Plant Operating Procedure I contains the Scaled Valve Check List which requires certain valves to be locked in position prior to plant startup.
System Operating Procedure 11 contains a Pre-operational u lve Check List for the Prestressed Concrete Reactor Vessel which requires proper valve positioning prior to plant startup.
Contrary to the above, two valves required to be sealed in position were found with no locking device and three valves required to be open were found to be closed with the reactor at power.
B.
Administrative Procedure (ADM) No. 28 requires that the person responsible write a Plant Trouble Report (PTR) if the recorder or integrator is not working properly.
Contrary to the above, the recorder for the " Ping" Monitor was noted not to be working on October 14, 1979, had not been working since September 22, 1979 and a PTR had not been written.
This is an infraction.
