Item 1 - Infraction.
Technical Specification 3.8.1.g requires a check of pri=ary coolant syste=
boron concentration each shift when refueling operatiens are being performed in the reactor.
Contrary to the above, the licensee failed to check primary coolant system boron concentration during the "A" shift on October 2,1979, a shift during which refueling operations vere being perfomed in the reactor.
Response to Item 1 Discussien The cpparent violation of Technical Specificatien 3.8.1.g is attributed to vo facter :

Corrective Action Taken and Results Achieved The refueling check sheets for the 1979 refueling outage have been examined, and it has been verified that the PCS boren concentratien during both the shifts ic=edie.tely preceeding and folleving "A" shift on October 2,1979, was in speci-fication. Accordingly, it is concluded that the PCS boren concentration during "A" shift on October 2,1979, was also within specifications.

December 22, 1979 Corrective Action to be Taken to Avoid Further Noncomoliance
'Ihis occurrence has been reviewed with the Chemistry Technicians, and the import-ance of obtaining required samples has been t,sressed. This event vill also be re-viewed with shift operating personnel, and the importance of verification of all refueling requiremnts during refueling operations vill be stressed.
Date When Full Comuliance Will be Achieved The reviews described above vill be completed prior to the next refueling outage.
Item 2 - Infraction Technical Specification 6.8.1.b requires implementation of procedures covering refueling eperatiens. Procedure FESO-2, " Refueling Procedure" includes checklist CL AS.lk-1, " Refueling Operations Shift Check Sheet," which states, in part:
"the minimum spent fuel pool / reactor cavity level is one foot below the surface skirrers during fuel handling."
Contrary to the above, the licensee handled fuel on the "A" shifts of October 11 and 12,1979, when the water level was 1.5 and 2 feet, respectively, below the skir=crs.
Resnonse to Item 2 Discussion The failure to properly i=plement Procedure FESO-2 resulted from a ec=bination of factors :
(1) a small leak in a steam generator dam combined with a small leak in the reactor cavity liner resulted in the pool level dropping more rapidly than vould be expected under normal refueling conditions; (2) the shift operating personnel failed to halt refueling operations upon discovery of this out of speci-fication condition.
Corrective Action Taken and Results Achieved Repairs have been made to both the leaking steam generator dam and the reactor cavity liner such that these leakage paths should be eliminated. Shift operat-ing personnel vill be instructed regarding the significant of verifying that prereq-uisites for refueling are within specifications.
Corrective Action to be Taken to Avoid Further Noncomoliance Shift operating personnel vill be trained in refueling procedures prior to Palisades next refueling outage.
Date When Full Comoliance Will be Achieved The required training vill be completed prior to the next refueling outage.
