Docket No. 50-458/79-04 Appendix A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on September 4-7, 1979, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Construction Permit No. CPPR-145 as indicated below:

10 CFR 50, Appendix B, Criterion V states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, er drawings.

A.

Failure to Control Ordering of Safety-Related Concrete The Gulf States QA Program, Section 17.1.5A of PSAR requires measures to be established to assure that activities affecting quality are prescribed by documented instructions, procedures, or drawings and accomplished in accordance with these instructions, procedures, or drawings.

The National Mobile Concrete Corporation (NMCC) Site QA Manual, Section 130, paragraph 137.1 requires that the location of placement be printed on each Batch Plant Ticket "as requested by the Order."

Contrary to the above; Use and control of the National Mobile Concrete Corporation (NMCC)

" Concrete Order" form is not prescribed in a documented instruction contained in either the licensee's, Stone and Webster's or NMCC's QA programs. As a result, inaccurate placment location information was printed on Batch Plant Tickets No. 00345,00346 and 00347 for safety-related concrete placement No. CDMM 6-3-3533 (Category I) in the Control Building on September 4, 1979.

This is an infraction.

Failure to Follow Manufacturer's Instructions for the Storage of Reinforcing Steel Mechanical Splice Sleeves Stone and Webster Specification No. 219.710, Revision 2, " Shop Fabrication and Field Erection of Reactor Containment Vessel Penetrations 1293 342 7 9110 8 0 2 Jc0?

and Hatches; Drywell Head Hatches and Steel Plate; and Shield Building Personnel Doors," states in part that, "The Cadweld sleeves shall be protected as recommended by the manufacturer."

Erico Products, Inc., Catalog RB10M974, "Cadweld Rebar Splicing,"

states in part, in the Section entitled " Care of Splice Sleeves, Cartridges and Equipment," that, " Corrosion protection should be kept intact until preparing for splicing."

Contrary to the above:

Fifteen B-series Cadweld sister sleeves were found on September 5, 1979, to be stored outdoors in the reinforcing steel storage yard without the rust inhibiting paper and Caplugs supplied by the manufacturer. Standing water was observed in the sleeves along with a small amount of corrosion.

This an infraction.
