This refers to your application dated September 26, 1979 requesting approval of your Quality Assurance (QA) program as meeting the QA program requirements of 10 CFR 571.51.
In connection with our review, we note that the organization chart shows a Radiation Safety Committee and a Radiation Control Officer while the text assigns responsibilities to the Radiation Safety Officer, Radiographers, and Radiography personnel. Please make the organization chart and the text consist-ent and resubmit seven copies within 30 days following receipt of this letter.
