Southwest Fabricating & Welding Co., Inc.
Docket No. 99900025/79-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on June 23-28, 1979, it appeared that certain of your activities were not conducted in full compliance with NRC requirement as indicated below:
A.
The Southwest Fabricating & Welding Co., Inc., corrective response letter of March 26 1979, states in part with respect to Item D. of Notice of Deviatiou, Inspection Report No. 79-01, ".
Issue a Notice of Pending Revision (NPR) approved by the Vice President - Engineering to authorize a Shop Copy Change to increase the forming temperature to 1750 F maximum from 1200 F maximum for this item..."
Contrary to the above, a Notice of Pending Revision was issued authorizing a Shop Copy Change of forming temperature, without obtaining the required approval of the Vice President - Engineering.
B.
Criterion V uf appendix B to 10 CFR 50 states, " Activities affecting
. quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be ac-complished in accordance with these inrtructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quan-titative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Paragraph 6.3.1 in Section 6 of the QA Manual states, "All welders and welding operators shall be qualified in accordance with applicable require-ments of Section III and Section IX.
Contrary to the above, on June 27, 1979, a welder was oh.,erved performing inconel buttering (operation 29, Sales Order S.O. Q4290, Sheet 77) who was not qualified in accordance with Section IX requirements.
