O Certain activities under your license appaar to be in violation of AEC regulations.
The follouing violations are considered to be of Category II severity.
1.
10 CFR 20.203(c)(2)(iii), " Caution signs, labels, signals and con-

trols", require that in the absence of certain control devices, each entrance or accuss poin: to a high radiition arca shall be r:aintained locked except during periods when access to the area is required with positive control over each individual entry.
Contrary to this requirement, on July 3, 1974, you failco to provide pocitive control over two individuals entering the Reactor Euilding which is maintained locked when access is not required.

Technical Specification 6.2.3 requires in part that " written procedures shall be strictly adhered to...."
Contrar/ to this requircnont, you did not follou certain procedures during the July 3, 1974, entry into the Reactor Building as noted belou:
a.
Section 6.4.1 of Health Physics Procedure 1630, Reactor Building Entry, requires that a radiation protection technician cust be included in the entry party.
A technician did not attend the entry.
b.
Radiation Work Permit (RL'P) 03'3, required that a film badge be worn.
One individual catered without a film badge.
c.
Section 5 of Security Procedure 1005.22.1 requires that certain individuds r.st be authorized on an access lict and must Ec cr.ct r : cd.
